Background Cardiovascular disease is the leading cause of death globally. Non-malignant gynaecological diseases (NMGD) significantly affect patient health and well-being may be associated with cardiovascular or cerebrovascular (C/CVD). Methods Seven databases were searched for relevant studies up to 21 April 2024. Observational reporting risk estimates 95% CIs association between NMGD C/CVD included. Polycystic ovary syndrome (PCOS) is a complex disorder with great heterogeneity of signs and symptoms. However, hyperandrogenism considered hallmark PCOS, presented by most affected women. Women PCOS are at high risk developing type 2 diabetes mellitus (T2DM), which associated insulin resistance (IR) hyperinsulinemia. In turn, hyperinsulinemia interferes the androgen production ovarian cells, worsens hyperandrogenism, initiating feedback cycle. also greater obesity. Indeed, dysfunctional adipose tissue in obesity contributes to T2DM affecting action secretion through multiple mechanisms, such as lipotoxicity, inflammation, adipokine signaling. Therefore, obesity-disrupted can be seen an important target for development women PCOS. Obesity increases the risk of anovulation, insulin resistance, hyperandrogenism, and endometrial dysfunction, resulting in women with infertility increasing preconceptional pregnancy complications. Bariatric surgery has been described as most effective intervention for obesity, improved fertility outcomes. However, its invasive nature potential nutritional deficiencies need a delayed conception post-surgery. On other hand, pharmacological treatments such glucagon-like-peptide 1 receptor agonists offer non-invasive alternatives promising results body weight, improving sensitivity restoring ovarian function. their use must be discontinued before due to fetal risks. Other available treatment options encompass topiramate, phentermine, Orlistat. The choice individualized considering cost-effectiveness, accessibility, obesity severity, reproductive goals, associated risks within each patient. A multidisciplinary approach is essential optimize metabolic health infertility. This review will examine impact on metabolism when comparing surgical interventions weight loss
