Introduction Temporary policy changes during the coronavirus disease 2019 pandemic facilitated rapid expansion of medication for opioid use disorder via telemedicine (tele-MOUD). Evidence tele-MOUD best practices and its impact on treatment engagement retention remains limited. This quality improvement initiative compared implementation among Pennsylvania (MOUD) programs, evaluated sociodemographic characteristics patients using tele-MOUD, described trends in patient retention. Methods Five health systems with MOUD programs completed questionnaires regarding their models provided aggregated data in-person visits 2020. Three monthly appointment (scheduled, completed, no-show, visits) over period which scaled up. Results Differences protocols related to provision inductions, eligibility operationalization remote drug testing. Across 88% prescribers conducted appointments, 50% used We observed differences, a greater proportion female, White, non-Hispanic tele-MOUD. data, overall enrollment increased, new remained relatively constant. Engagement suggested temporary decline no-show appointments that aligned escalation one program. Conclusions Tele-MOUD protocol differences indicate need research inform evidence-based guidance. Abstract Buprenorphine is a highly effective treatment for opioid use disorder and critical tool addressing the worsening U.S. overdose crisis. However, multiple barriers to - including stringent federal regulations have historically made this medication hard reach many who need it. In 2020, under COVID-19 Public Health Emergency, regulators substantially changed access buprenorphine by allowing prescribers initiate patients on via telehealth without first evaluating them in person. As Emergency set expire May of 2023, Congress agencies can leverage extensive evidence from studies conducted during wake pandemic make evidencebased decisions regulation going forward.
