ABSTRACT Objective This study aimed to identify risk factors associated with the development of metastases in breast cancer patients, investigate survival rates, and relationship between local recurrences distant metastases. Methods retrospective case‐cohort included women who were treated at a certified Breast Unit 2001 2015. Cases developed compared controls based on diagnosis year, stage, age diagnosis. Comprehensive information patient characteristics, tumor biology, treatment options was gathered. Results The 412 patients 433 remained metastasis‐free over median follow‐up 150 months (interquartile range 87–202). 20‐year overall 99.23% for control group 23.62% those metastasis ( p < 0.01). Significant lobular invasive carcinoma (odds ratio (OR) 2.26, 0.001), triple‐negative subtype (OR 4.06, = 0.002), high grade 2.62, 0.004), larger size 1.02, lymph node involvement loco‐regional recurrence 4.32, 0.001). Progesterone receptor (PR) expression protective 0.52, 95% confidence interval 0.34–0.81, 0.003). Machine learning models supported these findings, though their clinical significance limited. Conclusions Lobular carcinoma, specific subtypes, grade, large size, involvement, are all significant metastasis, whereas PR is protective. potential machine predicting explored, showing promise future personalized assessment.
