Multifocal meningoencephalitis after vaccination against COVID‐19

Abstract We report the case of an 84‐year‐old male patient who was transferred to our hospital because impaired consciousness and high fever, died about 10 weeks after his fourth “coronavirus disease 2019” (COVID‐19) vaccination. Autopsy revealed acute ischemic change with microhemorrhage perivascular T‐cell infiltration in thalamus, pons, cerebellum, which were considered be related neurological symptoms. There dilatation right ventricle, accumulation pleural effusion, ascites, suggesting heart failure. Although had a negative COVID‐19 polymerase chain reaction test, immunohistochemical analysis for severe respiratory syndrome coronavirus 2 (SARS‐CoV‐2) antigens (spike nucleocapsid proteins) performed identify cause death. Surprisingly, only SARS‐CoV‐2 spike protein detected pituitary adrenal glands. The presence might have been due vaccination rather than viral infection, no detected. central nervous system change, that glands may associated failure, possibly through dysfunction renin‐angiotensin‐aldosterone system.
