Abstract Cognitive impairment and function post-acute mild to moderate COVID-19 are poorly understood. We report findings of 128 prospectively studied SARS-CoV-2 positive patients. Cognition olfaction were assessed at 2-, 4- 12-months post-diagnosis. Lung function, physical mental health 2-month post diagnosis. Blood cytokines, neuro-biomarkers, kynurenine pathway (KP) metabolites measured 4-, 8- 12- months. Mild cognitive (demographically corrected) was present in 16%, 23%, 26%, diagnosis, respectively. Overall performance mildly, but significantly (p<.001) declined. more common those with anosmia (p=.05), only 2 KP quinolinic acid, 3-hydroxyanthranilic associated decline.
