Introduction Our study aims to provide a comprehensive overview of mesenchymal stem cell-derived extracellular vesicles (MSC-EVs) in drug delivery research, focusing on the period between 2013 and 2023. Given increasing global interest this field, we utilized bibliometric tools explore publication trends, key contributors, thematic research clusters. Methods Data was collected from Web Science (WoS) database, an in-depth analysis conducted using VOSviewer. The encompassed bibliographic coupling, co-citation, co-authorship, co-occurrence offering structured insight into activity. We also employed Citespace further analyze clusters domain. Results revealed total 1,045 publications related MSC-EVs over past decade, showing steady increase output. China led count, H-index, prolific authors, funding, while United States ranked highest citations, average citation counts, H-index performance. Pharmaceutics emerged as leading journal by volume, with Journal Controlled Release having strongest link strength. Top institutions driving included Shanghai Jiao Tong University, Zhejiang Harvard University. VOSviewer identified four major clusters: tissue engineering, cancer, neurological diseases, targeted delivery. refined ten areas, including differentiation, regeneration, resistance.
