Hepatitis E virus (HEV) infection is a frequent cause of acute viral hepatitis. Immunocompromised patients, especially those under anti-CD20 regimens, are prone to chronic or treatment-resistant courses hepatitis E. We report case HEV in 36-year-old man with history thrombotic thrombocytopenic purpura treated rituximab 6 months ago, who presented new-onset painless jaundice and malaise. Laboratory tests imaging revealed signs inflammation hepatic dysfunction. Due initial suspicion autoimmune hepatitis, corticosteroid therapy was started. However, liver biopsy positive RNA value redefined the diagnosis. Serology initially infection, which later progressed infection. Hepatitis E virus (HEV) can cause self-limiting acute and chronic hepatitis infections, particularly in immunocompromised individuals. In developing countries, HEV is mainly transmitted via drinking contaminated water, whereas zoonotic transmission dominates the route of infection developed including Japan.
