The efficacy of upper limb (UL) robot-assisted therapy (RAT) on functional improvement after stroke remains unclear. However, recently published randomized controlled trials have supported its potential benefits in enhancing the activities daily living, arm and hand function, muscle strength. Task-specific high-intensity exercises are key points facilitating motor re-learning neurorehabilitation since RAT can provide an assisted-as-needed approach. This study aims to investigate clinical effects exoskeleton robotic system for UL rehabilitation compared with conventional (CT) people subacute stroke. As a secondary aim, we seek identify patients' characteristics, which predict better recovery UL-RAT detects whether it could elicit greater brain stimulation.A total 84 patients will be recruited from 7 Italian centers over 3 years. randomly allocated either CT (control group, CG) or plus UL-RT through Armeo®Power (Hocoma AG, CH, Volketswil, Switzerland) (experimental EG). A sample stratification based distance onset, DSO (DSO ≤ 30; > 30), Fugl-Meyer Assessment (FM)-UL (FM-UL 22; 22 < FM-UL 44) considered randomization. outcomes recorded at baseline (T0), 25 + sessions intervention (T1), 6 months post-stroke (T2). functioning assessed by (0-66) primary outcome. assessments set International Classification Function, Disability Health (ICF). patient satisfaction questionnaire evaluated EG T1. subgroup T0 T1 via electroencephalography. Attention deficit-hyperactivity disorder (ADHD) is a neurodevelopmental with high incidence in children and adolescents characterized by motor hyperactivity, impulsivity, inattention. Magnetic resonance imaging (MRI) has revealed that neuroanatomical abnormalities such as the volume reduction neocortex hippocampus are shared several neuropsychiatric diseases schizophrenia, autism spectrum ADHD. Furthermore, abnormal development postnatal pruning of dendritic spines neocortical neurons intellectual disability well documented. Dendritic dynamic structures exhibiting Hebbian homeostatic plasticity triggers intracellular cascades involving glutamate receptors, calcium influx remodeling F-actin network. The long-term potentiation (LTP)-induced insertion postsynaptic receptors associated enlargement spine heads depression (LTD) shrinkage. Using murine model ADHD, delay spines’ maturation CA1 hippocampal correlated impaired working memory LTP recently reported. aim this review to summarize recent evidence emerged from studies focused on genetic features found ADHD patients reports animal models describing molecular structure spines.

The anterior dorsolateral striatum (DLS) is heavily innervated by convergent excitatory projections from the primary motor (M1) and sensory cortex (S1) considered an important site of sensorimotor integration. M1 S1 corticostriatal synapses have functional differences in their connection strength with striatal spiny projection neurons (SPNs) fast-spiking interneurons (FSIs) DLS and, as a result, exert distinct influences on sensory-guided behaviors. In present study, we tested whether inputs exhibit subcellular anatomical distribution neurons. We injected adeno-associated viral vectors encoding spaghetti monster fluorescent proteins (sm.FPs) into male female mice used confocal microscopy to generate 3D reconstructions single identified SPNs FSIs obtained through ex vivo patch clamp electrophysiology. found that dually innervate FSIs; however, there consistent bias towards input not FSIs. addition, were distributed similarly across proximal, medial, distal regions SPN FSI dendrites. Notably, closely localized clusters more prevalent than FSIs, suggesting cortical are integrated cell-type specific mechanisms. Our results suggest stronger connectivity compared S1, previously observed, due higher quantity synaptic inputs. implications for how integration performed cell-specific connections.
