Awareness theory posits that individuals connected to a brain-computer interface can learn estimate and discriminate their brain states. We conducted a prospective, single arm, multisite, multinational, open label trial assessing the safety and efficacy of novel amygdala derived neurofeedback treatment, designated Amygdala-Derived-EFP, for chronic PTSD. Participants, including veterans civilians, underwent screening, training, 15 sessions over 8 weeks and; baseline, termination (8 weeks) 3 month post treatment assessments with validated measures. The primary endpoint was more than 50% participants demonstrating Minimally Clinically Important Difference (MCID) defined as 6-point reduction, on Clinician Administered PTSD Scale (CAPS-5) total score at months. Secondary measures included PCL-5, ERQ, PHQ-9, CGI. Statistical analyses were performed using SAS®V9.4. met, CAPS-5 MCID response rate 66.7%. average reduction in scores follow up 13.5 points, twice MCID. Changes from baseline CAPS-5, PHQ-9 follow-up demonstrated statistically significant improvements effect sizes ranging 0.46 to 1.07. Adverse events mild resolved after treatment. This study builds prior research similar outcomes amygdala-derived neurofeedback. Abstract Hyperactivation of amygdala is a neural marker for post-traumatic stress disorder (PTSD) and improvement in control over activity has been associated with treatment success PTSD. In this randomized, double-blind clinical trial we evaluated the efficacy real-time fMRI neurofeedback intervention designed to train following trauma recall. Neurofeedback (NF) has emerged as a promising avenue for demonstrating process-related neuroplasticity, enabling self-regulation of brain function. Background: Post-traumatic stress disorder (PTSD) manifests through distinct symptom clusters that can respond differently to treatments. Neurofeedback guided by the Amygdala-derived-EEG-fMRI-Pattern (Amyg-EFP-NF) has been utilized train PTSD patients regulate amygdala-related activity and decrease symptoms. Methods: We conducted a combined analysis of 128 from three clinical trials Amyg-EFP-NF evaluate effects across (as assessed CAPS-5 subscales) on emotion regulation processing (evaluated ERQ). Results: significantly reduced severity all immediately post-treatment, with improvements maintained at three-month follow-up. Background To date, only one systematic review and meta-analysis of randomized controlled trials (RCTs) has evaluated the effect neurofeedback in PTSD, which included four studies found an uncertainty EEG-NF on PTSD symptoms. Neuroimaging technologies such as brain–computer interfaces and neurofeedback have evolved rapidly new tools for cognitive neuroscience potential clinical interventions.
