Background Acquired Immune Deficiency Syndrome (AIDS) combined with tuberculosis (TB) is one of the key factors affecting global TB control, and timely effective treatment essential to improve prognosis in this population. However, data from WHO have shown that patients AIDS a lower anti-TB success rate than HIV-negative individuals do, which may lead an increased incidence relapse drug resistance. Therefore, exploring risk outcome developing relevant predictive models will help clinicians rapidly identify at greater failure, highly valuable for clinical management. Methods We conducted retrospective cohort study including inpatients pulmonary (PTB) who were treated Beijing Youan Hospital between January 2020 2024. The baseline laboratory test all enrolled collected electronic medical records system. randomly divided participants into training set validation ratio 2:1 established LASSO Cox model on basis treatment. selected prognostic then used construct final model, was visualized using nomogram. receiver operating characteristic (ROC) curves, concordance index (C-index), calibration curves evaluate discrimination ability consistency respectively. Decision curve analysis (DCA) assess applicability models. Patients subsequently stratified according optimal cutoff value by X-tile software better decision-making clinicians. Results A total 203 PTB study, 141 (69.5%) 62 (30.5%) unfavorable outcome. results regression revealed CRP/albumin (CAR), extrapulmonary disseminated tuberculosis, other infectious diseases, cavitation independent outcomes PTB, whereas CD4 + T-cell counts protective factor patient outcomes. five variables further establish AUC (0.760 0.811 set) C-index (0.765 0.768 showed we constructed had good ability. indicated high predictions actual observations both set. DCA nomogram applicability. risk-stratified score, three groups: low (total points <358), medium (358 ≤ <373), ≥373). Clinicians should focus whose score more 358 points. Conclusion identified failure PTB. Our performed satisfactorily can be screening management high-risk patients.

Objective To investigate the relationship between C-reactive protein and albumin ratios (CAR) all-cause cardiovascular disease(CVD)-specific mortality in individuals with coronary heart disease(CHD). Methods The data from 1895 patients were extracted National Health Nutrition Examination Survey (NHANES) database 1999–2010. We used weighted COX regression analyses to explore association CAR, all-cause, CVD-specific mortality. Restricted cubic spline(RCS) models threshold effects analysis analyze nonlinear relationships. Subgroup also performed these relationships further. Results During a mean follow-up of 115.78 months, 61.48% deaths occurred, 21.85% due CVD. After adjusting for potential confounders, each 1-unit increase CAR was associated 65% 67% RCS model revealed non-linear risk CHD (all P < 0.001). Threshold identified inflection points (0.04, 0.001) (0.05, = 0.0024). interaction tests found sex, smoking diabetes influenced HF its 0.05). Conclusion There CVD CHD, higher hazard ratio before point. Sex, smoking, diabetes, might have an effect on associations death risks.
