BackgroundChildhood maltreatment is associated with adverse health outcomes and this risk can be transmitted to the next generation. We aimed investigate association between exposure maternal childhood common physical mental problems, neurodevelopmental disorders, related comorbidity patterns in offspring.Methods.We conducted a retrospective cohort study using data from Environmental influences on Child Health Outcomes (ECHO) Program, which was launched influence of early life exposures child development 69 cohorts across USA. Eligible mother–child dyads were those available at least one outcome measure (autism spectrum disorder, attention-deficit hyperactivity disorder [ADHD], internalising obesity, allergy, asthma diagnoses). Maternal history obtained retrospectively Adverse Childhood Experiences or Life Stressor Checklist questionnaires. derived prevalence specified measures offspring adolescence by harmonising caregiver reports other relevant sources (such as medical records) cohorts. symptoms assessed Behavior Checklist. Associations measured series mixed-effects logistic regression models. Covariates included sex (male female), race, ethnicity; paternal age; education; combined annual household income; diagnosis depression, asthma, ADHD, autism disorder; obesity. Two latent class analyses conducted: characterise outcomes; co-occurrence subtypes. then investigated membership outcomes, respectively.FindingsOur sample 4337 21 longitudinal (with collection initiated 1999 2016). Of 3954 mothers study, 1742 (44%) had experienced abuse neglect during their childhood. After adjustment for confounding, who more likely have children problems clinical range (odds ratio [OR] 2·70 [95% CI 1·95–3·72], p<0·0001), (1·70 [1·13–2·55], p=0·01), ADHD (2·09 [1·63–2·67], (1·54 [1·34–1·77], p<0·0001). In female offspring, higher obesity (1·69 [1·17–2·44], p=0·005). Children exposed exhibit diagnostic pattern characterised multimorbidity. Exposure multiple forms all subtypes highest increases most suggesting dose–response relationship.InterpretationOur findings suggest that experiences factor disease susceptibility variety emphasise need policies focusing breaking intergenerational transmission adversity.FundingEnvironmental Office Director, National Institutes Health.

Intergenerational trauma, violence, and maltreatment, in which symptoms or experiences of an ancestor's trauma repeat otherwise manifest subsequent generations, presents a weighty societal challenge to multiplicity therapeutic intervention strategies have been applied. Theoretical perspectives are antecedent clinical social intervention, informing decisions both policy practice. However, these frequently remain subliminal imperceptible the discourse, resulting interventions that somewhat dislocated from their theoretical foundations. This narrative review seeks summarize discuss each theories as they apply intergenerational reveal potential association with specific models approaches. It positions flexibility between integration opportunities reach new enhanced understandings engender distinctive interventions. An enriched understanding explaining deeper appreciation for pertinence theory practice, incitement blend unique ways is, herewith, reached.

Abstract Exposure to traumatic events during pregnancy may influence and birth outcomes. Growing evidence suggests that exposure well before pregnancy, such as childhood maltreatment (CM), also the course of risk adverse We aimed estimate associations between maternal CM small-for-gestational-age (SGA) preterm (PTB) in a diverse US sample, examine whether common CM-associated health behavioral sequelae either moderate or mediate these associations. The Measurement Maternal Stress (MOMS) Study was prospective cohort study enrolled 744 healthy English-speaking participants ≥ 18 years with singleton who were < 21 weeks at enrollment, 2013 2015. measured via Childhood Trauma Questionnaire (CTQ) above moderate/severe cut-off for any five abuse neglect scales assigned CM-exposed group. Common (obesity, depressive symptoms, hypertensive disorders) (substance use) obtained from standardized questionnaires medical records. main outcomes included PTB (gestational age 37 birth) SGA (birthweight 10%ile gestational age) abstracted record. Multivariable logisitic regression used test CM, sequeale, outcomes, both moderation mediation by CM-related tested. Data available 657/744 participants. Any reported 32% Risk 61% higher among those group compared non-CM (14.1% vs. 7.6%), each subsequent form an individual exposed corresponded 27% increased (aOR 1.27, 95% CI 1.05, 1.53). There no significant association (9.3% 13.0%, aOR 1.07, 0.58, 1.97). Of only disorders associated did not SGA. Our findings indicate is highlight importance investigating mechanisms whereby adversity sets trajectory long-term intergenerational issues.

Numerous preclinical and clinical studies have shown that stress is one of the main environmental factor playing a significant role in pathogeny life-course bowel diseases. However, stressful events occur early life, even during fetal leave different traces within central nervous system, area involved response autonomic network but also emotion, cognition memory regulation. Early-life can disrupt prefrontal-amygdala circuit thus favoring an imbalance system hypothalamic-pituitary adrenal axis, resulting anxiety-like behaviors. The down regulation vagus nerve cholinergic anti-inflammatory pathway favors pro-inflammatory conditions. Recent data suggest emotional abuse at life are aggravating risk factors inflammatory disease. This review aims to unravel mechanisms explain consequences pathophysiology disease their mental co-morbidities. A therapeutic potential will be covered.

Abstract Exposure to traumatic experiences across lifespan shapes the functioning of hypothalamic pituitary adrenal (HPA) axis and sets individuals at risk develop symptoms depression anxiety. Particularly, HPA regulation psychological health expectant mother have been interest, as unborn child may be affected through changes in gestational biology. The present study investigated potential associations between lifetime trauma, current (depression anxiety) hair cortisol concentrations (HCC) pregnant women. A total 149 women were interviewed public outpatient clinics with varying age Greece, Spain Perú. Lifetime trauma exposure anxiety assessed. HCC was measured scalp-near segments (2 cm length) reflecting cumulative secretion past two months. Results showed that load is negatively associated higher There a negative association symptoms. findings support notion exerts long-lasting effects on mother’s activity mental thereby potentially create trajectories for via

Background Children of maltreated mothers are at increased risk for adverse physical and psychological health. Both prenatal postnatal alterations in offspring biological stress systems have been proposed as mechanisms contributing to such transmission. The aim the current study was assess whether maternal care infant moderated any effect childhood maltreatment on cortisol output during a mild stressor 4 months age. Methods Participants included 181 mother–infant dyads, screened recruitment result 57.4% reporting one or more forms maltreatment.
