In this prospective study, we evaluated the diagnostic yield and safety of two endobronchial ultrasound (EBUS) biopsy techniques – mediastinal cryobiopsy (EBUS-MCB) Franseen tip needle (EBUS-ANB) in patients with undiagnosed lymphadenopathy. The study included 30 who underwent both EBUS-MCB EBUS-ANB, four biopsies taken from each patient using methods. results demonstrated that provided a higher (96.4%) compared to EBUS-ANB (73.3%). Specimens showed fewer artifacts density granulomas were adequate for ancillary studies all cases. most common complication observed was minor bleeding, which more (36.6% versus 13.3%, p=0.04). This demonstrates EBUS-guided has when have an acceptable profile. Larger comparing these are necessary confirm findings current study.

ABSTRACT The current standard for determining sample adequacy during Endobronchial Ultrasound guided Mediastinal Sampling is ROSE (Rapid on‐site Evaluation) on samples obtained by Fine needle Aspiration. There an increasing interest in Lymph node cryobiopsy, where tissue are using a 1.1 mm cryoprobe. Rapid onsite evaluation cryobiopsy can also be performed imprint cytology. This proof‐of‐concept study explores the use of Viva Scope 2500 rapid digital histopathological (EBUS) mediastinal lymph samples. Introduction: Endobronchial ultrasound (EBUS)-guided transbronchial mediastinal cryobiopsy (TBMC) is increasingly used to diagnose lymphadenopathy. Various methods have been create a tunnel between the airway wall and lesions for this procedure, such as electrocautery penetration with sheath of needle EBUS-transbronchial fine aspiration. However, those are complex. Case Presentation: We developed new technique called EBUS-TBMC via tunnel, we it in four cases and/or hilar puncture dilation catheter target lymph node. Abstract EBUS-guided transbronchial mediastinal cryobiopsy (TBMC) has emerged as a promising biopsy tool for diagnosing hilar and pathologies. However, several fundamental technical aspects of TBMC remain unexplored. This study aims to determine the optimal number cryo-passes freezing time ultrathin cryoprobe in EBUS-TBMC concerning specimen size procedural diagnostic yield. We conducted retrospective chart review patients with lesions who underwent between January 2021 April 2023 across three hospitals Malaysia. A total 129 procedures were successfully completed, achieving an overall yield 88.4%. Conclusive associated larger sizes (7.0 vs. 5.0 mm, p < 0.01). Specimen demonstrated positive correlation ( 0.01), plateauing at 4.1–6.0 mm. significant was also observed both 0.01) 0.05). Diagnostic plateaued after 2–3 cryo-passes. In contrast, longer times trended towards smaller specimens lower yield, though not reaching statistical significance. Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is a revolutionary tool for the diagnosis and staging of mediastinal disorders. Introduction: Endobronchial ultrasound-guided transbronchial needle aspiration remains the gold standard for diagnosis of mediastinal pathologies. Its greatest limitation has been low diagnostic yield in lymphoproliferative disorders as well insufficient samples molecular testing. Transesophageal examinations using an EBUS scope have helped increase by allowing additional biopsies paraesophageal and intra-abdominal lesions. Similarly, novel approach cryobiopsy further increased providing larger better-preserved biopsies. Both complimentary techniques shown great individual feasibility safety. However, transesophageal cryobiopsies evaluation pathologies a subject debate. Aim: The aim study was to investigate safety EBUS-guided performed at our center. Methods: We conducted retrospective review 30 patients who underwent through esophagus institution between October 2023 March 2024. Data on patient demographics, yield, complications were collected analyzed. Results: mean age 43 years, with gender distribution 60% male 40% female. primary indications included suspicion disorders, suspected sarcoidosis, malignancies overall 93%. This narrative review examines the existing literature on minimally invasive image-guided sampling techniques of mediastinal lesions gathered from international databases (Medline, PubMed, Scopus, and Google Scholar). Original studies, systematic reviews with meta-analyses, randomized controlled trials, case reports published between January 2009 November 2023 were included. Four authors independently conducted search to minimize bias, removed duplicates, selected evaluated studies.
