Abstract Mitochondria, which are important mediators for cancer initiation, growth, metastasis, and drug resistance, have been considered as a major target in therapy. Herein, an acid‐activated mitochondria‐targeted nanocarrier is constructed precise delivery of nitric oxide (NO) adenosine triphosphate (ATP) suppressor to amplify the therapeutic efficacy treatments. By combining α‐cyclodextrin (α‐CD) acid‐cleavable dimethylmaleic anhydride modified PEG conjugated mitochondria‐targeting peptide, shows prolonged blood circulation time enhanced cellular uptake together with selectively restoring capability under tumor extracellular pH (6.5). Such specific NO proves crucial inducing mitochondria dysfunction through facilitating mitochondrial membrane permeabilization downregulating ATP level, can inhibit P‐glycoprotein‐related bioactivities formation tumor‐derived microvesicles combat resistance metastasis. Extensive research has been undertaken in the pursuit of anticancer therapeutics. Many drugs require specificity delivery to cancer cells, whilst sparing healthy tissue. Cell-penetrating peptides (CPPs), now well established as facilitators intracellular delivery, have recent years advanced incorporate target and thus possess great potential for targeted cargoes. Though none yet approved clinical use, this novel technology already entered trials. In review we present CPPs, discuss their classification, mechanisms cargo internalization highlight strategies conjugation moieties including incorporation into therapeutic proteins.
