Acute
kidney
injury
(AKI)
is
a
life-threatening
condition
characterized
by
rapid
and
transient
decrease
in
function.
AKI
part
of
an
array
conditions
collectively
defined
as
acute
diseases
(AKD).
In
AKD,
persistent
damage
dysfunction
lead
to
chronic
disease
(CKD)
over
time.
A
variety
insults
can
trigger
AKI;
however,
chemotherapy-associated
nephrotoxicity
increasingly
recognized
significant
side
effect
chemotherapy.
New
biomarkers
are
urgently
needed
identify
patients
at
high
risk
developing
subsequent
AKI.
However,
lack
understanding
cellular
mechanisms
that
chemotherapy-related
has
hindered
the
identification
effective
date.

ABSTRACT
Chronic
kidney
disease
(CKD)
will
become
the
fifth
global
case
of
death
by
2040.
Its
largest
impact
is
on
premature
mortality
but
number
persons
with
failure
requiring
renal
replacement
therapy
(RRT)
also
increasing
dramatically.
Current
RRT
suboptimal
due
to
shortage
donors
and
dismal
outcomes
associated
both
hemodialysis
peritoneal
dialysis.
Kidney
care
needs
a
revolution.
In
this
review,
we
provide
an
update
emerging
knowledge
technologies
that
allow
earlier
diagnosis
CKD,
addressing
current
so-called
blind
spot
(e.g.
imaging
biomarkers),
improve
therapies
(wearable
artificial
kidneys,
xenotransplantation,
stem
cell-derived
therapies,
bioengineered
bio-artificial
kidneys).

Although
the
prognosis
of
lupus
nephritis
(LN)
has
improved
over
last
few
decades,
5-20%
patients
still
progress
to
kidney
failure.
Hence,
there
is
an
unmet
need
improve
management
LN.
Two
novel
drugs,
belimumab
and
voclosporin,
have
been
recently
approved
for
LN
obinutuzumab
in
late
stage
development.
In
randomised
controlled
trials
(RCTs),
all
these
added
standard-of-care,
were
more
effective
than
standard-of-care
alone
achieving
renal
response.
Now
question
is:
should
new
drugs
be
used
early
disease
course
or
just
refractory
patients?
The
main
reasons
supporting
use
are
based
on
RCTs
that
demonstrated
benefits
when
combinatory
regimen
was
initiated
incident
relapsing
leading
a
higher
proportion
achieve
response,
hence
reducing
nephron
loss
risk
regimens
primarily
relapsing/refractory
acknowledge
many
responded
well
even
without
add-on
medications,
allowing
economic
innovative
costly
drugs.
However,
good
predictors
response
lacking
and,
thus,
decision
adding
treatments
consider
drug
access,
risks
undertreatment,

substantively
differ
predicting
outcomes.

prediction.

10-point
score
using
any
outperformed
based
on
>50%
discriminating
