Multiple
sclerosis
(MS)
is
a
chronic
autoimmune
disease
affecting
the
central
nervous
system
(CNS),
characterized
by
inflammation
and
neurodegeneration.
The
pathophysiology
of
MS,
especially
its
progressive
forms,
involves
various
cellular
components,
including
microglia,
primary
resident
immune
cells
CNS.
This
review
discusses
role
microglia
in
neuroinflammation,
tissue
repair,
neural
homeostasis,
as
well
their
involvement
MS
explores
potential
therapeutic
strategies
targeting
microglial
function.
A
literature
search
conducted
August
2023
updated
March
2025,
using
PubMed
database,
focused
on
articles
relating
to
published
2018-2025.
Additionally,
ongoing
clinical
trials
Bruton's
tyrosine
kinase
(BTK)
inhibitors
were
identified
through
ClinicalTrials.gov
website
November
2025.
Microglia
are
highly
adaptive
exhibit
functional
states
throughout
different
life
stages
play
critical
roles
homeostasis.
Their
altered
activity
prominent
feature
contributing
pathogenesis.
Imaging
techniques
such
magnetic
resonance
imaging
(MRI)
positron
emission
tomography
(PET)
provide
insights
into
MS.
BTK
other
novel
treatments
for

show
promise
modulating
function
influencing
progression
rate.

To
determine
the
prognostic
value
of
persisting
neuroinflammation
in
multiple
sclerosis
(MS)
lesions,
we
developed
a
18
kDa-translocator-protein-positron
emission
tomography
(PET)
-based
classification
each
lesion
according
to
innate
immune
cell
content
and
localization.
We
assessed
respective
predictive
phenotype
diffuse
inflammation
on
atrophy
disability
progression
over
2
years.Thirty-six
people
with
MS
(disease
duration
9
±
6
years;
12
relapsing-remitting,
13
secondary-progressive,
11
primary-progressive)
19
healthy
controls
(HCs)
underwent
dynamic
[18
F]-DPA-714-PET.
At
baseline
after
years,
patients
also
magnetic
resonance
imaging
(MRI)
neurological
examination.
Based
threshold
significant
defined
by
comparison

between
HCs,
white
matter
lesions
were
classified
as
homogeneously
active
(active
center),
rim-active
(inactive
center
periphery),
or
nonactive.
