Postmenopausal women are overrepresented in the preserved ejection heart failure population. Expansion of visceral and epicardial adipose tissue during menopause transition leads to local low-grade systemic inflammation that turn contributes left ventricular concentric remodeling, diastolic dysfunction development progression fraction. Objectives A systematic review and meta-analysis was conducted to validate the effects of resistance training (RT) on body composition physical function in older females with sarcopenic obesity (SO). Design Systematic meta-analysis. Setting participants Older (≥60 years). Methods Four electronic databases—PubMed, Web Science, Embase, Cochrane Library—were comprehensively searched through June 2024. Randomized controlled trials (RCTs) comparing RT non-exercise interventions or health education were included. Outcomes measured included key indicators such as function. The quality studies evaluated using Physiotherapy Evidence Database (PEDRO) score, risk bias assessed utilizing Risk Bias 2.0 Tool (RoB 2). Ultimately, a RevMan 5.4. Results our revealed that partially ameliorated patients, significantly reducing fat percentage (BF%; WMD = −2.83, 95% CI: −4.55 −1.12, p 0.001). However, comparative analysis control groups, we it did not influence other indices mass index (BMI; −0.42, −1.92 1.08, 0.58), total skeletal muscle (TSM; −0.62, −2.38 1.15, 0.49), bone mineral density (BMD; 0.01, −0.03 0.05, 0.68). Notably, demonstrated substantial efficacy enhancing function, evidenced by improvements 10-meter walk test (10WMT; 0.22 s, 0.04 0.39, 0.01), Timed Up Go (TUG; −2.23 −2.96 −1.49, 0.00001), Chair Rise (TCR; 5.20 repetitions, 3.98 6.43, 0.00001). Conclusion This indicates exerts significant positive SO. Despite these benefits, impact parameters, BF%, appears be limited. These findings underscore need for further investigation into mechanisms which affects this patient population. registration INPLASY202430061 https://inplasy.com/inplasy-2024-3-0061/ .
