Abstract Growing evidence suggests an increasing significance for the extent of gastrointestinal tract (GIT) dysfunction in Parkinson’s disease (PD). Most patients suffer from GIT symptoms, including dysphagia, sialorrhea, bloating, nausea, vomiting, gastroparesis, and constipation during course. The underlying pathomechanisms this α-synucleinopathy play important role development progression, i.e., early accumulation Lewy pathology enteric central nervous systems is implicated pharyngeal discoordination, esophageal gastric motility/peristalsis impairment, chronic pain, altered intestinal permeability autonomic colon, with subsequent constipation. Severe complications, malnutrition, dehydration, insufficient drug effects, aspiration pneumonia, obstruction, megacolon, frequently result hospitalization. Sophisticated diagnostic tools are now available that permit more detailed examination specific impairment patterns. Furthermore, novel treatment approaches have been evaluated, although high-level trials often missing. Finally, burgeoning literature devoted to microbiome reveals its importance neurologists. We review current knowledge about pathoanatomy, pathophysiology, diagnosis, PD provide recommendations management daily practice.

Despite clinical evidence of poor oral health and hygiene in Parkinson's disease (PD) patients, the mouth is often overlooked by both patients medical community, who generally focus on motor or psychiatric disorders considered more burdensome. Yet, a two-way relationship with overall health-a weakened status triggering decline quality life. Here, we aim at giving comprehensive overview PD, while identifying their etiologies consequences. The physical (abnormal posture, muscle tone, tremor, dyskinesia), behavioral (cognitive neuropsychiatric disorders), iatrogenic patterns associated PD have an detrimental effect patients' health, putting them risk for other (infections, aspiration, pain, malnutrition), reducing life increasing isolation (anxiety, depression, communication issues). Interdisciplinary cooperation prevention, management follow-up strategies need to be implemented early stage maintain improve comfort condition. Recommendations practice, including (non-)pharmacological are discussed, emphasis neurologists' role. Of interest, cavity may become valuable tool diagnosis prognosis near future (biomarkers). This but critical issue requires further attention interdisciplinary research.

Parkinson's disease (PD) is characterized by the progressive loss of dopaminergic neurons in substantia nigra, mainly affecting people over 60 yr age. Patients develop both classic symptoms (tremors, muscle rigidity, bradykinesia, and postural instability) nonclassical (orthostatic hypotension, neuropsychiatric deficiency, sleep disturbances, respiratory disorders). Thus, patients with PD can have a significantly impaired quality life, especially when they do not multimodality therapeutic follow-up. The alterations associated this syndrome are main cause mortality PD. They be classified as peripheral caused disorders upper airways or muscles involved breathing central triggered functional deficits important located brainstem control. Currently, there little research describing these disorders, therefore, no well-established knowledge about subject, making treatment difficult. In review, history pathology data changes obtained thus far will addressed.

Abstract Background The etiologies of chronic neurological diseases, which heavily contribute to global disease burden, remain far from elucidated. Despite available umbrella reviews on single contributing factors or no study has systematically captured non-purely genetic risk and/or protective for diseases. Methods We performed a systematic analysis ( meta-umbrella ) published until September 20th, 2018, using broad search terms in MEDLINE, SCOPUS, Web Science, Cochrane Database Systematic Reviews, Cumulative Index Nursing and Allied Health Literature, ProQuest Dissertations & Theses, JBI Reviews Implementation Reports, DARE, PROSPERO. PRISMA guidelines were followed this study. Reference lists the identified also screened, methodological details assessed AMSTAR tool. For each factor association, random effects summary effect size, 95% confidence prediction intervals, significance heterogeneity levels facilitated assessment credibility epidemiological evidence identified. Results 2797 potentially relevant reviews, 14 (203 unique meta-analyses) eligible. median number primary studies per meta-analysis was 7 (interquartile range (IQR) 7) that participants 8873 (IQR 36,394). yielded 115 distinctly named non-genetic with significant various strengths evidence. Mediterranean diet associated lower dementia, Alzheimer (AD), cognitive impairment, stroke, neurodegenerative diseases general. In Parkinson (PD) AD/dementia, coffee consumption, physical activity factors. Low serum uric acid increased PD. Smoking elevated multiple sclerosis dementia but PD, while hypertension PD higher dementia. Chronic occupational exposure lead amyotrophic lateral sclerosis. Late-life depression AD any form Conclusions several preventive clinical neurology, health policy, lifestyle counseling. Our findings could offer new perspectives secondary research meta-research ).

The aim of this study was to determine healthcare providers' knowledge and practices about dysphagia. A descriptive cross-sectional carried out based on a self-administered anonymous questionnaire addressed providers in Spain. total 396 participated the study. Of these, 62.3% knew definition dysphagia as swallowing disorder. In addition, up 39.2% participants reported that they did not know whether EatingAssessmentTool (EAT-10) screening test usedin their own clinical settings. Similarly, 49.1% them ClinicalExaminationVolume-Viscosity (MECV-V) method. A neurogenic dysphagia is caused by problems with the central and peripheral nervous systems, particularly prevalent in conditions such as Parkinson’s disease stroke. It significantly impacts quality of life for affected individuals causes additional burdens, malnutrition, aspiration pneumonia, asphyxia, or even death from choking due to improper eating. Physical therapy offers a non-invasive treatment high efficacy low cost. Evidence supporting use physical increasing, including techniques neuromuscular electrical stimulation, sensory transcranial direct current repetitive magnetic stimulation. While initial studies have shown promising results, effectiveness specific regimens still requires further validation. At present, there lack scientific evidence guide patient selection, develop appropriate regimens, accurately evaluate outcomes. Therefore, primary objectives this review are results existing research, summarize application management, we also discussed mechanisms treatments dysphagia.

Gastrointestinal (GI) issues are commonly experienced by patients with Parkinson’s disease (PD). Those that affect the lower GI tract, such as constipation, most frequently reported problems among PD. Upper issues, swallowing dysfunction (dysphagia) and delayed gastric emptying (gastroparesis), also common in PD but less well recognized both clinicians and, therefore, often overlooked. These may be perceived healthcare team of a priority than management motor symptoms. However, if left untreated, dysphagia gastroparesis can have significant impact on quality life effectiveness oral medications, negative consequences for control. Holistic should therefore include timely effective upper utilizing non-pharmacological pharmacological approaches. This dual approach is key many strategies limited efficacy this setting, so approaches best option. Although multidisciplinary to ideal, resource constraints mean not always feasible. In ‘real-world’ practice, neurologists care teams need make initial assessments treatment or referral recommendations their who experiencing these problems. To provide guidance cases, article reviews published evidence diagnostic therapeutic gastroparesis, including appropriate specialists when needed development an plan.
