Cognitive functioning can be negatively influenced by age, cardiovascular risk (CVR) and mental health challenges, sex-hormones have neuroprotective effects. Little is known about cognitive in older transgender individuals receiving long-term gender-affirming hormone therapy (GHT). In a previous, smaller study, differences between women cisgender groups were minimal yet statistically significant. This study assessed larger samples of individuals, the contribution CVR mental/social to these differences. cross-sectional compared 73 39 men (56-84 y) GHT (10-47 with matched (age; education level) from Longitudinal Aging Study Amsterdam on neuropsychological tests. Mean z-scores per domain calculated analyzed using linear regression. Models subsequently adjusted for ((history of) disease; smoking) (anxiety; loneliness) factors. Transgender had lower scores than men, respectively, information-processing speed (b = -0.62, 95% CI -0.90 -0.35; b -0.33, 95%CI -0.60 -0.05), episodic memory -1.28, -1.53 -1.04; -0.77, -1.01 -0.52), crystallized intelligence -0.42, -0.75 -0.10; -0.41, -0.08). scored but equal -0.43, -0.79 -0.08; -0.01, -0.36 0.35). Mental/social factors (particularly depressive symptoms) largely, slightly, explained trans- groups. Small do not suggest adverse or beneficial testosterone effects functioning. However, groups, which was largely health. warrants further research clinical awareness individuals.
