Abstract Background MicroRNAs (miRNAs) are small non-coding RNAs that post-transcriptionally regulate gene expression. Perturbations in plasma miRNA levels known to impact disease risk and have potential as biomarkers. Exploring the genetic regulation of miRNAs may yield new insights into their important role governing expression mechanisms. Results We present genome-wide association studies 2083 circulating 2178 participants Rotterdam Study identify miRNA-expression quantitative trait loci (miR-eQTLs). 3292 associations between 1289 SNPs 63 miRNAs, which 65% replicated two independent cohorts. demonstrate miR-eQTLs co-localise with expression, protein, metabolite-QTLs, help identifying miRNA-regulated pathways. investigate consequences alteration on a wide range clinical conditions phenome-wide Mendelian randomisation using UK Biobank data ( N = 423,419), revealing pleiotropic causal effects several various conditions.
