This study aims to investigate alterations in immune cell counts within preovulatory follicles of patients with poor ovarian response (POR) during assisted reproductive technology (ART), classified according the POSEIDON criteria. single-centre cross-sectional included 543 women undergoing IVF/ICSI treatment, selected based on specific inclusion and exclusion criteria: 292 normal 251 response. Follicular fluid (FF) was collected day oocyte retrieval analysed by flow cytometry determine proportions macrophages (Mφs), M1 M2 Mφs, T cells (CD4 CD8 cells), dendritic (DCs), including type 1 conventional (cDC1) 2 (cDC2), neutrophils. Multivariable logistic regression assessed relationship between POR, Pearson correlation determined associations number retrieved oocytes, receiver operating characteristic (ROC) curves evaluated predictive power for POR. Immune accounted 52.57% (±23.90%) total population follicular microenvironment, which approximately equal that granulosa cells, Mφs being most abundant, followed sequentially DCs, In overall infiltration microenvironment decreased, whereas polarization increased. increased, a decrease CD4/CD8 ratio. Both cDC1 cDC2 were significantly elevated. Moreover, multivariable revealed macrophage count, CD4 count independent predictors Notably, showed largest area under ROC curve, suggesting its strong potential as biomarker predicting The proportion altered These findings suggest dynamics may play crucial role determining prognosis, indicating targeted immunomodulatory strategies could be considered future therapeutic approaches.

Introduction The mechanisms leading to ovarian primordial follicle depletion following gonadotoxic chemotherapy with cyclophosphamide and other cytotoxic drugs are currently understood through two main explanatory theories: apoptosis over-activation. Discrepancies between the findings of different studies investigating these do not allow reach a firm conclusion. heterogeneity cell types in ovaries their degrees sensitivity damage, cell-cell interactions, periodical profile differences, model age-dependent differences exposure durations tested may partially explain discrepancies among studies. Methods This study used intact prepubertal mice culture as model, which most follicles follicles. Histological transcriptional analyses exposed active metabolite 4-hydroperoxycyclophosphamide (4-HC) were carried out via time-course experiment at 8, 24, 48, 72 h. Results 4-HC treated showed significant decrease density 24 h, along DNA damage (TUNEL) overexpressed signals (cleaved-poly ADP ribose polymerase immunohistochemistry western blotting). Meanwhile treatment significantly up-regulated H2ax, Casp 6, Noxa, Bax ovaries, Puma (FISH). Discussion Our results indicated that cyclophosphamide-induced acute was mainly related apoptotic pathways. No evidence activation found, neither changes expression genes nor growing Further validation protein level 4-HC-treated h confirmed observations.
