Itch is a topic to which everyone can relate. The physiological roles of itch are increasingly understood and appreciated. pathophysiological consequences impact quality life as much pain. These dynamics have led deep dives into the mechanisms that underlie contribute sensation itch. When prior review on physiology itching was published in this journal 1941, black box interest small number neuroscientists dermatologists. now appreciated complex colorful Rubik’s cube. Acute chronic being carefully scratched apart reassembled by puzzle solvers across biomedical spectrum. New mediators identified. Mechanisms blur boundaries circuitry blend neuroscience immunology. Measures involve psychophysics behavioral psychology. efforts associated with these approaches positively impacting care itchy patients. There potential markedly alleviate itch, condition does not end life, but often ruins it. We field provide current understanding pathophysiology disease, only symptom disease.

Pruritus is a common symptom of several skin diseases, both inflammatory and neoplastic. might have tremendous impact on patients’ quality life strongly interfere with sleep, social, work activities. We review the role type-2 inflammation immunity in pathogenesis chronic pruritic conditions skin. Type 2 cytokines, including IL-4, IL-13, thymic stromal lymphopoietin, periostin, IL-31, IL-25, IL-33 are released by mast cells, innate lymphoid cells 2, keratinocytes, type T lymphocytes, master regulators itch. These cytokines act as direct pruritogen primary sensory neurons (pruriceptors) or alter sensitivity to other itch mediators inflammation- immunity-dominated atopic dermatitis, prurigo nodularis, bullous pemphigoid, scabies, parasitic urticaria, Sézary syndrome indeed associated most severe pruritus. In contrast, such scleroderma, lupus erythematosus, hidradenitis suppurativa, acne, less represented, pruritus milder variable. Th2 evolved protect against parasites, thus, scratching response evoked developed alert about presence remove parasites from surface.

Abstract Itch is a defining symptom of atopic dermatitis. Crosstalk between keratinocytes, the immune system and non‐histaminergic sensory nerves responsible for pathophysiology chronic itch in An expanding understanding contribution nervous its interaction with pathways are helping to identify new therapeutic strategies.
