Abstract Craniopharyngioma (CP) is an intracranial tumor with high mortality and morbidity. Though biologically benign, CP will damage the hypothalamus, inducing comorbidities such as obesity, metabolic syndrome, cognitive impairments. The roles of gut microbiome serum metabolome in CP‐associated hypothalamic are aimed to be explored. Patients characterized by increased Shannon diversity, Eubacterium , Clostridium Roseburia alongside decreased Alistipes Bacteroides . CP‐enriched taxa positively correlated dyslipidemia decline, while CP‐depleted negatively associated fatty liver. Subsequent metabolomics identified notably up‐regulated purine metabolism, integrative analysis indicated association between altered microbiota elevated hypoxanthine. Phenotypic study multi‐omics Rax‐CreER T2 ::Braf V600E/+ ::Pten Flox/+ mouse model validated potential involvement dysregulated metabolism comorbidities.
