Active matrix metalloproteinase-8 (aMMP-8) is a promising biomarker candidate for the modern periodontal and peri-implant disease diagnostics utilizing chairside/point-of-care oral fluid technologies. These rapid analysis technologies utilize gingival crevicular (GCF), sulcular (PISF), or mouth rinse as matrices that can be collected patient-friendly non-invasively without causing bacteremia. aMMP-8, but not total latent proMMP-8, has been shown to relevant implemented latest 2017 classification system of periodontitis peri-implantitis. Abstract Objective The aim of this cross-sectional study was to investigate the relationship between periodontitis, potential periodontitis oral fluid biomarkers, and prediabetes. Diabetes and periodontitis, as widespread chronic diseases, often exacerbate each other's conditions. Nonsurgical periodontal treatments can improve the oral systemic health in these patients, with salivary proteins offering potential insights into disease mechanisms treatment effectiveness. However, there is a lack of comprehensive data on proteomics this context. By assigning patients diabetes-associated periodontitis to test group (supragingival scaling, subgingival scraping root planing) or control scaling only), analyzing nonstimulated whole saliva samples using liquid chromatography-tandem mass spectrometry, establishing vivo vitro models, we found significant differential expression related Apelin signaling pathway, hematopoietic cell profiling, stress response immune regulation, identifying four candidate proteins: superoxide dismutase 1 (SOD1), profilin (PFN1), S100 calcium-binding protein A11 (S100A11) kallikrein-related peptidase 6 (KLK6). All were significantly elevated, combined area under curve 0.933, while SOD1 alone reached 0.915. Additionally, observed high glucose inflammatory conditions reduced expression.
