Active matrix metalloproteinase-8 (aMMP-8) is a promising biomarker candidate for the modern periodontal and peri-implant disease diagnostics utilizing chairside/point-of-care oral fluid technologies. These rapid analysis technologies utilize gingival crevicular (GCF), sulcular (PISF), or mouth rinse as matrices that can be collected patient-friendly non-invasively without causing bacteremia. aMMP-8, but not total latent proMMP-8, has been shown to relevant implemented latest 2017 classification system of periodontitis peri-implantitis. Thus, aMMP-8 point-of-care-testing (POCT)-but proMMP-8-can conveniently used an adjunctive preventive diagnostic tool identify screen developing ongoing breakdown well predict its episodic progression. Similarly, POCT provides important monitor treatment effect these diseases, also other diseases such head neck cancer, where it tissue destructive side-effects during after radiotherapy. Additionally, recent studies support benefitting identification diabetes escalating risk COVID-19 infection. Overall, launched new clinical field in medicine dentistry, i.e., chemistry.

Abstract Objective The aim of this cross-sectional study was to investigate the relationship between periodontitis, potential periodontitis oral fluid biomarkers, and prediabetes. Materials Methods This included 150 Greek adults aged 25 78 years who were tested with an Hemoglobin A1C (HBA1c) diagnostic system, active-matrix metalloproteinase-8 (aMMP-8) point-of-care (PoC) test, several salivary biomarkers enzyme-linked immunosorbent assay tests gelatin zymography. A full-mouth clinical examination performed assess their periodontal health status. Statistical Analysis Kruskal–Wallis test used determine statistically significant difference in levels different stages, grades, stages grades combined. Spearman's rank correlation strength direction association aMMP-8 HbA1c (<5.7 ≥5.7%) other among patients severe periodontitis. two-sided p-value below 0.05 considered study. Results aMMP-8, but not total MMP-8 or associated significantly stage grade combined (p < 0.001, test). Among III C patients, positively correlated prediabetes (Spearman's rho = 0.646, p 0.044), (rho 0.636, 0.048), PMN Elastase 0.729, 0.017), MMP-9 0.721, 0.019), MMP-8/TIMP-1 molar ratio 0.879, 0.001). Conclusion Prediabetic disease development can upregulate expression (total MMP-8) rapidly progressing, where latent species are further activated into active forms (aMMP-8). Simultaneously, proinflammatory biomarker elevated tissue-destructive cascade. is easily detectable online/in real-time within 5 minutes by PoC testing at dentist's office.

Abstract Objective To examine the characteristics of salivary microbiome in Type 2 diabetes mellitus (T2DM) patients with or without periodontitis. Background Periodontitis has been identified as clear sequelae T2DM. This chronic oral disease also impacts management clinical features diabetes. The T2DM and periodontitis, well response this to nonsurgical therapy have not described. Knowledge key biological could help address observed poorer presentation patients. Methods saliva adult cohorts periodontally healthy/non‐diabetic (non‐periodontitis; NP; n = 31), periodontitis (DWoP; 32), (DWP; 29) were characterized by microbial molecular analysis using V3–V4 sequencing Luminex ELISA techniques for host analytes. Results Phyla distribution showed DWP significantly lower levels Firmicutes Actinobacteria higher Fusobacteria Spirochetes compared healthier groups. Approximately 10% detected species significant differences frequency level colonization among DWP, DWoP, NP samples. A subset bacteria correlated features, a specific repertoire analytes, particular matrix metalloproteinase (MMP)8/MMP9, interleukin‐1ß, B‐cell activating factor, resistin differed between groups related taxa. Principal component that majority subjects was unique based upon an array 27 taxa out up 255 Conclusion show analyte composition diabetics non‐diabetic persons Specific members relate directly and/or biomolecules individuals.

Diabetes and periodontitis, as widespread chronic diseases, often exacerbate each other's conditions. Nonsurgical periodontal treatments can improve the oral systemic health in these patients, with salivary proteins offering potential insights into disease mechanisms treatment effectiveness. However, there is a lack of comprehensive data on proteomics this context. By assigning patients diabetes-associated periodontitis to test group (supragingival scaling, subgingival scraping root planing) or control scaling only), analyzing nonstimulated whole saliva samples using liquid chromatography-tandem mass spectrometry, establishing vivo vitro models, we found significant differential expression related Apelin signaling pathway, hematopoietic cell profiling, stress response immune regulation, identifying four candidate proteins: superoxide dismutase 1 (SOD1), profilin (PFN1), S100 calcium-binding protein A11 (S100A11) kallikrein-related peptidase 6 (KLK6). All were significantly elevated, combined area under curve 0.933, while SOD1 alone reached 0.915. Additionally, observed high glucose inflammatory conditions reduced expression. In conclusion, emerges promising regulatory target for managing diabetes by modulating oxidative microenvironment.

ABSTRACT Aim Salivary content is regarded as a powerful diagnostic window for oral and systemic diseases the proteomic profile could be useful to distinguish between different periodontal conditions. The aim of present systematic review was assess distinctive salivary proteins identified through untargeted proteomics in periodontitis patients compared periodontally healthy gingivitis subjects, well provide qualitative methodological assessment current literature. Methods Relevant studies from Medline via PubMed, Scopus, Embase, Cochrane Library databases were retrieved answer following PECO question: “In systemically individuals, are there any differences protein expression profiles assessed with or subjects?” Moreover, utility markers sought targeted literature search further quantitative assessment. A modified version QUADAOMICS tool used quality included studies. Results After screening 461 relevant articles, total 13 selected. number discriminant ranged 2 4161. However, it possible identify that consistently over‐ under‐expressed at least 3 Among these, complement C3, profilin‐1, SA100A8, fibrinogen reported increased periodontitis, while cystatin‐SN leukocyte elastase inhibitor more elevated controls. Only 4 accuracy measures, SA100A8 showing an area under curve 0.71 (95% CI: 0.66–0.75) meta‐analysis. Conclusions Untargeted techniques some key biological molecules which periodontitis. These findings support novel candidate biomarkers high level heterogeneity methods reporting urge develop standardized protocols implemented this research field (PROSPERO CRD42022299826).

Type 2 diabetes mellitus (T2DM) is associated with cellular abnormalities, tissue and organ dysfunctions, periodontitis. This investigation examined the relationship between oral microbiome salivary biomarkers in T2DM patients or without cohort (35-80 years) included systemically healthy non-periodontitis (NP; n = 31), periodontitis (DWoP; 32) (DWP; 29). The [Operational Taxonomic Units (OTUs)] (16 s rRNA sequencing) targeted host (immunoassays) were assessed. We identified 47 OTUs that significantly different abundance NP samples any disease subset subgroups. most unique patterns observed DWP group. Differences genera/species also when stratified by extent of periodontal inflammation (i.e., generalized versus localized gingivitis/periodontitis). Salivary showed significant elevations MMP-8, MMP-9, resistin, IL-1β, IL-6, IFNα, BAFF (THFSR13b) comparing to analytes positive correlations specific members, predominantly patients. Odds ratio analyses reinforced a panel biologic markers (IL-6, MMP-8) bacteria (e.g., Bacteroidetes, Fusobacteria, Spirochaetes) discriminated severity this diabetic population.
