Research Review: The impact of the COVID‐19 pandemic on the mental health of children and young people with pre‐existing mental health and neurodevelopmental conditions – a systematic review and meta‐analysis of longitudinal studies

Background
Systematic
reviews
have
suggested
mixed
effects
of
the
COVID‐19
pandemic
on
mental
health
children
and
young
people.
However,
most
included
studies
focused
general
population
were
cross‐sectional.
The
long‐term
impact
those
with
pre‐existing
and/or
neurodevelopmental
conditions
remains
unclear.
Thus,
we
conducted
a
systematic
review
meta‐analysis
to
examine
longitudinal
this
clinical
potential
explanatory
factors.
Methods
Ovid
Medline,
Embase,
APA
PsycInfo
Global
Health
databases
searched
between
1
January
2020
3
August
2023
(PROSPERO
CRD42022383546).
We
that
compared
symptoms
pre‐
during
timepoints
in
people
(≤18
years
old)
conditions.
Outcomes
internalising,
externalising
other
symptoms.
Risk
bias
was
rated
using
an
adapted
tool.
Included
narratively
synthesised
multi‐level
meta‐analyses
where
number
sufficient.
Results
identified
21
(
N
=
2,617)
from
6,083
records.
Studies
differed
across
countries,
diagnoses,
measures,
informants
timepoints.
All
had
overall
moderate‐to‐high
risk
bias.
Narrative
synthesis
found
evidence
symptom
change,
individual
showing
increase/reduction/no
change.
Factors
such
as
diagnosis,
baseline
severity,
age
sex/gender
may
explain
variation
outcomes.
Multi‐level
feasible
for
limited
outcomes
no
significant
changes
internalising
versus
or
phases,
high
heterogeneity
noted.
Conclusions
varied
according
contextual
vulnerabilities,
which
not
fully
captured
pooled
analyses.
Further
research
needs
investigate
longer‐term
impacts
better
stratify
vulnerable
population.

Background
The
literature
is
equivocal
as
to
whether
the
predicted
negative
mental
health
impact
of
COVID-19
pandemic
came
fruition.
Some
quantitative
studies
report
increased
emotional
problems
and
depression;
others
improved
well-being.
Qualitative
explorations
reveal
heterogeneity,
with
themes
ranging
from
feelings
loss
growth
development.
Objective
This
study
aims
analyze
free-text
responses
children
young
people
participating
in
Children
Young
People
With
Long
COVID
get
a
clearer
understanding
how
were
feeling
during
pandemic.
Methods
A
total
8224
analyzed
using
InfraNodus,
an
artificial
intelligence–powered
text
network
analysis
tool,
determine
most
prevalent
topics.
random
subsample
411
(5%)
underwent
manual
sentiment
analysis;
this
was
reweighted
represent
general
population
England.
Results
Experiences
fell
into
6
main
overlapping
topical
clusters:
school,
examination
stress,
health,
pandemic,
social
family
support,
physical
(including
symptoms).
Sentiment
showed
that
statements
largely
(314/411,
76.4%),
small
proportion
being
positive
(57/411,
13.9%).
Those
reporting
mostly
female
(227/314,
72.3%),
while
those
older
(170/314,
54.1%).
There
significant
observed
associations
between
status
well
sex
(P=.001

respectively)

Objectives
The
COVID-19
pandemic
has
disproportionately
impacted
vulnerable
groups’

and
mental

Phenomenological
Analysis,

indicated
that
participants’
health
was
negatively
by
pandemic,
feelings
loneliness
being
most
common
experience.
However,
positive
effects
concurrently
observed
including
improved
well-being
better
coping
strategies
post
lockdown,
which
testament
people’s
resilience.
That
said,
it
clear
backgrounds
lacked
during
now
psychological,
practical
relational
assistance
challenges.
Conclusions
While
future
studies
benefit
larger
ethnically
diverse
sample,
start.
Study
findings
have
potential
inform
government
policies
around
access
for
notably
prioritising
grassroots
initiatives
