Adenoid cystic carcinoma (ACC) of the salivary glands is second most common type gland cancer, and characterized by a poor prognosis an unclear pathology. The incidence ACC rare, as it accounts for 10-15% all tumors affects mainly patients aged between 50 60 years. annual rate estimated to be ~4.5 cases per 100,000 individuals. Due its rarity use contaminated cell lines in previous investigations, precise etiological factors underlying remain poorly understood. Current treatment modalities, typically involving surgery with or without postoperative radiotherapy, often prove unsatisfactory due potential local recurrence delayed distant metastases, which may manifest 3-5 years after constitute primary failure existing therapeutic approaches. indolent growth pattern, along perineural perivascular invasion, potentially responsible onset metastases. No effective systemic therapy has been established so far. Therefore, management represents significant challenge. Adenoid cystic carcinoma (ACC) is a relatively uncommon tumor among head and neck cancers, with laryngeal involvement, typically subglottic, being exceptionally rare. While ACC usually originates in the salivary glands, its occurrence larynx highly unusual. Laryngeal adenoid (LACC) characterized by indolent growth, frequent perineural invasion, tendency for local recurrence. Moreover, distant metastasis can occur lungs most common site of metastasis. We present case 28-year-old male patient progressively enlarging right mass, uncovering rare infiltrative LACC invading thyroid gland. Post thyroidectomy, 18-fluoro-2-deoxyglucose (F-18-FDG)-positron emission tomography (PET)/computed (CT) scan revealed FDG-avid posterior subglottic residual disease invasion into adjacent structures, including trachea. This represents one few documented cases LACC, contributing to sparse literature on this aggressive malignancy. highlights diagnostic challenge clinical significance FDG-PET/CT evaluating malignancies like LACC.

ABSTRACT Background Adenoid cystic carcinoma (ACC) is a slow‐growing tumor with variable outcomes, making follow‐up protocols challenging. This review analyzed outcomes in ACC, focusing on patterns and factors related to adverse prognoses. Methods A search was conducted March 25, 2024, MEDLINE, PubMed, Scopus, EMBASE. Studies analyzing ACC survival were included, while those lacking these data or single site excluded. The Joanna Briggs Institute's Critical Appraisal tool used assess the risk of bias, metanalysis performed using R software. Results Among 57 studies (17 497 patients), predominantly affected women (mean age: 52.69 years) occurred mainly minor salivary glands. Distant metastasis most frequent outcome, occurring at 34.8 months average. Locoregional local recurrence 27.5 31.3 months, respectively. Five‐, 10‐, 15‐, 20‐year rates 73.8%, 48.5%, 42.3%, 26.5%, Poor predictors included advanced T stage, positive lymph nodes, solid tumors, distant metastasis, surgical margins. Conclusions outcome prognostic are residual tumors.

Adenoid cystic carcinoma (ACC) is a rare salivary gland tumor that accounts for approximately 1% of all head and neck cancers. Despite its initial indolent behavior, long-term survival poor due to locoregional recurrence in 40% distant metastasis up 60% patients who undergo radical treatment. The histological parameters ACC the combination these histopathological grading systems provide valuable prognostic information about clinical course disease. Within this context, review aims analyze impact parameters, individual or combined malignancy, on prognosis. Individual such as solid pattern, presence necrosis, high-grade transformation, dominance epithelial component, perineural lymphovascular invasion, positive surgical margins have negative impacts with ACC. There are currently four ACC; however, few studies validated most them explored small cohorts short follow-up. Considering application has been associated prognosis, broader validation will allow not only their use prediction but also assist treatment planning.

BackgroundSalivary gland carcinomas (SGCs) are exceedingly rare in children, with a reported annual incidence of 0.8-1.4/1,000,000 under 20 years age. Evidence regarding optimal treatment pediatric SGCs is limited, and for long time, no guidelines have been available. Here, we report on an international retrospective series children adolescents collected by several national tumor study groups cooperating the European Cooperative Study Group Pediatric Rare Tumors (EXPeRT)Patients MethodsPatients diagnosed between 2000 2014 were included Data reviewed respective working harmonized data sheet to EXPeRT central analysis.ResultsOverall, 121 patients identified, including 103 parotid tumors, 12 submandibular tumors six minor glands. In 11 patients, secondary cancers. Mucoepidermoid carcinoma was most frequent diagnosis (n=65), followed acinic cell (n=39), adenocystic (n=7), sialoblastoma (n=3) other (n=7). All underwent resection (R0: 66%, R1: 34%). Neck dissection performed 47 revealing nodal metastases 13. Twenty-four irradiation, received adjuvant chemotherapy. During median follow-up 25 (6-140 months), 14 relapses observed (7 local, 5 2 distant metastases). Five died disease. Higher histological grade associated advanced local stage risk recurrence.ConclusionsSGCs mostly present as localized low malignant potential. approximately 10% regional lymph node at diagnosis. After complete resection, prognosis favorable. Surgery mainstay treatment; or cervical irradiation should be reserved those less favorable biology such carcinoma.
