Abstract Psychiatric neuroimaging faces challenges to rigour and reproducibility that prompt reconsideration of the relative strengths limitations study designs. Owing high resource demands varying inferential goals, current designs differentially emphasise sample size, measurement breadth, longitudinal assessments. In this overview perspective, we provide a guide landscape psychiatric with respect balance scientific goals constraints. Through heuristic data cube contrasting key design features, discuss resulting trade-off among small sample, precision studies (e.g., individualised cohorts) large minimally longitudinal, population studies. Precision support tests within-person mechanisms, via intervention tracking course. Population generalisation across multifaceted individual differences. A proposed reciprocal validation model (RVM) aims recursively leverage these complementary in sequence accumulate evidence, optimise strengths, build towards improved long-term clinical utility.

Attention-deficit/hyperactivity disorder (ADHD) is a biologically and clinically heterogeneous neurodevelopmental condition, which hinders the identification of rooted evidence for treatment choices clinical predictions. Identifying brain-based homogenous ADHD subtypes with neuroimaging data to reduce this heterogeneity promising elucidating specific neural mechanisms underlying complex presentations, may enable development personalized treatments precise therapeutic targets. In review, we first discuss large individual differences among patients indicated by findings from both large-scale group-level studies individual-level studies, motivated new efforts discover neurobiological subtypes. Next, review recent research on neuroimaging-based in terms three aspects: sample selection, subtyping methodology (i.e., features, algorithms, validation strategies), subtype findings. Eleven utilizing multiple single modalities or multimodal were identified. Through diverse features approaches, current have revealed range different characterized distinct profiles, providing important insight into nature ADHD. Despite progress, most still little biological relevance, limited utility, generalizability, slowing down pace their translation. We highlight several crucial considerations overcome these challenges contribute more useful reproducible identification. With increasing access datasets, deliberate features/methods adequate strategies, believe that could be used inform treatments, thereby advancing practice towards precision psychiatry.

Stimulant drugs that boost dopamine, like methylphenidate (MP), enhance attention and are effective treatments for attention-deficit hyperactivity disorder (ADHD). Yet there is large individual variation in attentional capacity response to MP. It unclear whether this driven by differences relative density of dopamine receptor subtypes, magnitude increases induced MP, or both. Here, we extensively characterized the brain system with positron emission tomography (PET) imaging (including striatal D1 D2/3 availability MP-induced increases) measured task-evoked fMRI activity two separate sessions (placebo 60 mg oral MP; single-blind, counterbalanced) 37 healthy adults. A network lateral frontoparietal visual cortices was sensitive increasing (and working memory) load, whose positively correlated performance across individuals (partial r = 0.474, P 0.008; controlling age). change within 0.686, < 0.001).
