Treatment rates and delays for mental and substance use disorders: results from the Australian National Survey of Mental Health and Wellbeing

Abstract
Aims
Prompt
initial
contact
with
a
treatment
provider
is
critical
first
step
in
seeking
help
for
mental
or
substance
use
disorders
(SUDs).
The
aim
of
the
current
study
was
to
provide
estimates
patterns
and
predictors
delay
making
based
on
recently
completed
Australian
National
Survey
Mental
Health
Wellbeing.
Methods
Data
came
nationally
representative
epidemiological
survey
n
=
15,893
Australians.
Measures
included
DSM-IV
lifetime
diagnoses
mood
(MD),
anxiety
(AD)
SUDs;
age
disorder
onset;
contact.
Correlates
were
examined.
Results
SUDs
exhibited
lowest
rate
(27%),
compared
MD
(94%)
ADs
(85%).
Individuals
AD
experienced
longest
(Mdn
11
years),
followed
by
those
8
years)
MDs
3
years).
Females
had
higher
odds
but
lower
SUDs.
Recent
birth
cohorts
showed
increased
across
disorders,
education
associated
AD.
Age
onset,
country
co-occurring
mixed
associations
seeking.
Conclusions
reveals
stark
disparities
treatment-seeking
behaviour
delays
pronounced
underutilization
services

The
high
level
of
psychological
distress
in
young
people
is
a
growing
concern.
However,
there
are
few
national
surveys
that
describe
the
trajectories
mental
health
and
wellbeing
through
adolescence
into
early
adulthood.
Further,
existing
research
has
largely
focused
exclusively
on
ill-health,
with
little
focus
positive
health.
This
study
provides
first
profile
Australians
aged
12-25
years.
Participants
completed
National
Youth
Mental
Health
survey
2018
(n1
=
3832),
2020
(n2
974)
or
2022
(n3
961).
We
applied
Keyes'
Complete
(CMH)
framework
to
derive
categories
wellbeing,
examine
rates
CMH
over
time,
by
age
gender.
While
approximately
half
those
surveyed
reported
flourishing
(high
without
illness),
declined
between
2022.
Rates
generally
decreased
age,
was
more
prevalent
amongst
males
than
females.
findings
provide
unique
contrast
youth
pre-,
during
post-
COVID-19
pandemic.

Background:
Previous
estimates
from
2007
found
that
co-occurring
mental
and/or
substance
use
disorders
were
a
pervasive
feature
of
Australia’s
health.
Since
time
there
have
been
shifts
and
improvements
in
the
conceptualisation
incorporation
research
treatment
settings.
The
current
study
provides
up-to-date
on
prevalence
disorders,
highlights
common
patterns
co-occurrence,
identifies
significant
correlates
examines
any
changes
extent
since
2007.
Methods:
Data
two
Australian
National
Surveys
Mental
Health
Wellbeing
conducted
2020–2022
(
N
=
15,893)

Descriptive
statistics
estimated

correlations
pairwise
conditional

Multinomial
logistic
robust
Poisson
regressions

to
identify
compare
conditions
across
surveys.
Results:
Approximately
46%
people
with

disorder
past
12
months
experienced
more
diagnosable
conditions.

was
little
evidence
suggest

changed
(Prevalence
Ratio
(PR)
1.08,
95%
CI
0.98–1.18).
Subgroup
analysis
indicated
those
aged
16–24
years
significantly
likely
experience
co-occurrence
compared
(PR
1.44,
1.17–1.77).
Conclusions:
Co-occurring
remain
endemic
Australia.
Indeed,
they
appear
be
increasingly
problematic
younger,
recent
cohorts.

transdiagnostic
interventions
