This guideline presents an update to the 2013 American College of Gastroenterology Guideline on Diagnosis and Management Celiac Disease with updated recommendations for evaluation management patients celiac disease (CD). CD is defined as a permanent immune-mediated response gluten present in wheat, barley, rye. has wide spectrum clinical manifestations that resemble multisystemic disorder rather than isolated intestinal disease, characterized by small bowel injury presence specific antibodies. Detection CD-specific antibodies (e.g., tissue transglutaminase) serum very helpful initial screening suspicion CD. Intestinal biopsy required most confirm diagnosis. A nonbiopsy strategy diagnosis selected children suggested discussed detail. Current treatment requires strict adherence gluten-free diet (GFD) lifelong medical follow-up. Most have excellent GFD. Nonresponsive persistent or recurrent symptoms despite being These require systematic workup rule out conditions may cause symptoms, especially unintentional contamination. Refractory rare nonresponsive often associated poor prognosis.

Celiac disease is an immune-mediated systemic triggered by intake of gluten in genetically susceptible individuals. The prevalence celiac the general population estimated to be 1% world. Its differs depending on geographical and ethnic variations. has increased significantly last 30 years due knowledge awareness physicians widespread use highly sensitive specific diagnostic tests for disease. Despite about disease, up 95% patients still remain undiagnosed. presentations have changed few decades. Classical symptoms occur a minority patients, while older children either minimal or atypical symptoms. Serologic should done with unexplained chronic intermittent diarrhea, failure thrive, weight loss, delayed puberty, short stature, amenorrhea, iron deficiency anemia, nausea, vomiting, abdominal pain, distension, constipation, recurrent aphthous stomatitis, abnormal liver enzyme elevation, who belong groups at risk. Early diagnosis very important prevent long-term complications. Currently, only effective treatment lifelong gluten-free diet. In this review, we will discuss epidemiology, clinical findings, tests, light latest literature.

In celiac disease, small intestinal transglutaminase 2 causes deamidation of glutamine residues in gluten peptides, which enhances stimulation T cells and leads to mucosal injury. Inhibition is a potential treatment for disease. In proof-of-concept trial, we assessed the efficacy safety 6-week with ZED1227, selective oral inhibitor, at three dose levels as compared placebo, adults well-controlled disease who underwent daily challenge. The primary end point was attenuation gluten-induced damage, measured by ratio villus height crypt depth. Secondary points included intraepithelial lymphocyte density, Celiac Symptom Index score, Disease Questionnaire score (for assessment health-related quality life).Of 41 patients assigned 10-mg ZED1227 group, 50-mg 100-mg 40 placebo 35, 39, 38, 30 patients, respectively, had adequate duodenal-biopsy samples point. Abstract In the present era, food scientists are concerned about exploiting functional crops with nutraceutical properties. Buckwheat is one of pseudocereals components used in treatment health‐related diseases, malnutrition, and celiac diseases. As a preferred diet as gluten‐free product for buckwheat good source nutrients, bioactive components, phytochemicals, antioxidants. The general characteristics better nutritional profile than other cereal family were highlighted by previous investigations. buckwheats, like peptides, flavonoids, phenolic acids, d ‐fagomine, fagopyritols, fagopyrins posing significant health benefits. This study highlights current knowledge its characteristics, constituents, their potential developing products to target people (1.4% world population)

A lifelong gluten-free diet (GFD) is the only treatment for celiac disease and other gluten-related disorders. Nevertheless, strict adherence to GFD often challenging due concerns about social isolation, risk of gluten contaminations, high cost, poor quality taste products. Moreover, although effective in achieving mucosal healing, it may lead dietary imbalances nutrient deficiencies over a long period time. To overcome these issues, several wheat flours have been developed create products that closely resemble their gluten-containing counterparts. Furthermore, given critical importance adhering GFD, becomes essential promote monitor possible voluntary or involuntary transgressions. Various methods, including clinical assessment, questionnaires, serology disease, duodenal biopsies detection Gluten Immunogenic Peptides (GIPs) are employed this purpose, but none considered entirely satisfactory. Since poses challenges, alternative therapies should be implemented coming years improve efficacy life patients with disease. The aim narrative review explore current knowledge investigate its future perspectives, focusing on technology advancements, follow-up strategies insights into rapidly changing future.
