Pneumocystis jirovecii pneumonia (PJP) is an opportunistic fungal infection that often occurs secondary to human immunodeficiency virus (HIV) infection. However, for non-HIV immunocompromised patients, such as those undergoing novel immunosuppressive treatments manage malignancies, organ transplants, or connective tissue diseases, PJP emerging increasing threat. The clinical manifestations of in HIV-infected and non-HIV-infected patients differ significantly. In progresses rapidly challenging diagnose, resulting severe respiratory failure a poor prognosis. We describe lymphocytopenia two women who were recently treated with methotrexate, tacrolimus, corticosteroids therapy following adjuvant chemotherapy breast cancer kidney transplantation. The initial examination included high-resolution chest CT indicating atypical pneumonia, treatment was initiated trimethoprim -sulfamethoxazole oxygen support.Subsequently, bronchoscopy bronchoalveolar lavage mNGS detected jirovecii. After 3 weeks cotrimoxazole, the recovered significantly their condition stable.

Asthma and COPD have characteristic symptoms, yet patients with both are prevalent. Despite this, there is currently no globally accepted definition for the overlap between asthma COPD, commonly referred to as asthma–COPD (ACO). This prospective observational took place Jordan between 1 September 2021 30 April 2022. Sputum samples from with recognized ACO were acquired within 48 h onset again at 3 weeks following exacerbation. next-generation sequencing Illumina MiSeq employed uncovered significantly high diversity sputa. results showed significant decrease taxonomic richness sputum collected episodes compared those (p = 0.008), increase evenness < 0.005). change composition airway community suggests that replacement portion certain microorganisms may play role microbial observed Greater knowledge link could allow for more focused administration antibiotics, especially exacerbations, improving clinical efficacy patient outcomes.

Introduction Pneumocystis jirovecii is an opportunistic, human-specific fungus that causes pneumonia (PCP). PCP symptoms are nonspecific. A patient with P. and another lung infection faces a diagnostic challenge. It may be difficult to determine which of these agents responsible for the clinical symptoms, preventing effective treatment. Diagnostic treatment efforts have been made more by rising frequency coronavirus 2019 (COVID-19) co-occur.Areas covered Herein, we provide comprehensive review pharmacological recommendations along literature in immunocompromised patients focusing on HIV-uninfected patients.Expert Opinion masked identifying co-existing pathogens not necessarily observed infection. Patients severe form COVID-19 should examined underlying immunodeficiency, co-infections must considered as co-infection worsen COVID-19ʹs severity fatality. investigated risk factors who come suggestive radiographic but previously received prophylaxis. prophylaxis explored individuals various conditions impair immune system, depending their risk.
