Abstract Purpose: Substantial clinical heterogeneity in older adults with diabetes makes therapeutic management challenging. Background: The timing of insulin administration, particularly for long-acting insulins like glargine 300 U/mL (Gla-300), plays a critical role in diabetes management. choice between daytime and nighttime administration Gla-300 is nuanced decision that should be tailored to each patient's unique circumstances. This study aims compare the efficacy safety profile initiation with when administered during day versus at night adult type 2 mellitus (T2DM) patients. Methods: A retrospective observational included records T2DM patients initiated on follow-up data three months. Cases complete medical records, including hemoglobin A1C (HbA1C) levels, fasting plasma glucose (FPG), post-prandial (PPPG), incidence hypoglycemic events, were included. Data pertaining glycaemic control safety, extracted categorized into two groups based administration. Records evaluated over months results statistically analysed. Results: total 169 patients, matched baseline characteristics, showed significant reduction indices from third month. However, intergroup difference was non-significant suggesting (morning vs. night) does not significantly impact effectiveness lowering glycemic indices. Incidences overall events numerically lesser those morning dosing 13.09% as compared 21.17%. Nocturnal hypoglycaemia 3.57% 7.05% observed. these differences insignificant (p> 0.05). Conclusion: Timing initiation, dosing, nocturnal lower Gla-300, though it significant. An individualized approach selection therapy essential optimizing minimizing hypoglycemia risk.

This study investigated the prevalence and clinical management of atherosclerotic cardiovascular disease (ASCVD) ASCVD risk in patients with type 2 diabetes (T2D) Jordan. PACT-MEA (NCT05317845) was a non-interventional, cross-sectional, observational adults T2D recruited seven countries across Middle East Africa. In Jordan, assessments were conducted at ten sites, three primary care secondary settings. This study explores the role of inflammation and oxidative stress, hallmarks COVID-19, in accelerating cellular biological aging. We investigated early molecular markers—DNA methylation age (DNAmAge) telomere length (TL)—in blood leukocytes, nasal cells (NCs), induced sputum (IS) one year post-infection pauci- asymptomatic healthcare workers (HCWs) infected during first pandemic wave (February–May 2020), compared to COPD patients, model for “aged lung”. Data from questionnaires, Work Ability Index (WAI), analyses, autonomic cardiac balance assessments, heart rate variability (HRV), pulmonary function tests were collected. El desarrollo de la civilización se asocia con un incremento en edad población. Hoy Argentina el adulto mayor (AM: persona 65 años o más) varón tiene una expectativa vida 76 y mujer 80 años, lo que representa 15% del porcentaje población los grandes centros urbanos. Se estima AM diabetes mellitus (DM) es entre 20-30%. Esta patología, sus comorbilidades complicaciones (en especial cardiovasculares, infecciosas, cognitivas oncológicas) llaman a esfuerzo equipo salud y, especial, las personas dedicadas este grupo etario. Es importante enfoque multidisciplinario, adaptado circunstancias su situación sociocultural económica, orientando recursos prevención primaria, secundaria terciaria. Frente al cambio constante, actualmente requiere participación para brindar respuestas soluciones donde control metabólico, cardiovascular, cognitivo movilidad tiendan mantener independencia funcional fin priorizar seguridad técnicas diagnósticas terapéuticas marco diálogo acuerdos.
