Critically ill patients frequently experience sleep deprivation, which has detrimental effects such as increased pain perception, impaired cognitive function, and delirium.Aim: Evaluate the effect of implementing nursing interventions bundle on quality associated delirium among at intensive care unit (ICU).Design: A quasi-experimental research design was utilized in this study.Setting: Anesthesia Intensive Care Unit Emergency hospital affiliated to Tanta University hospital.Subjects: Purposive sampling 60 adult critically who admitted selected setting divided into two equal groups; 30 each (control study groups).Tools; Tool (I) Patient interview schedule, tool (II) Richard Campbell Sleeping Questionnaire (RCSQ) (III) Delirium Screening Checklist (ICDSC) Results: highly significant difference detected between control groups regarding their total after applying intervention ( P=0.000).The incidence 30% group compared 13.33% seventh day with differences (P=0.024).Conclusion: Implementing can improve decrease delirium.Recommendation: Sleep should be integrated routine for ICU promote patient reduce delirium.

Clinical sequelae and symptoms for a considerable number of COVID-19 patients can linger months beyond the acute stage SARS-CoV-2 infection, “long COVID”. Among long-term consequences cognitive issues (especially memory loss or “brain fog”), chronic fatigue, myalgia, muscular weakness resembling myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) are importance. Melatonin may be particularly effective at reducing signs infection due to its functions as an antioxidant, anti-inflammatory, immuno-modulatory agent. is also chronobiotic medication in treating delirium restoring circadian imbalance seen COVID intensive care unit. Additionally, cytoprotector, melatonin aids prevention several comorbidities, including diabetes, metabolic syndrome, ischemic non-ischemic cardiovascular diseases. Abstract Because the chronobiotic and cytoprotective molecule melatonin diminishes with age, its involvement in postmenopausal senescence pathology has been considered since long. One relevant target site aging individuals is bone where effects mediated by MT1 MT2 receptors are demonstrable. Precursors of cells located marrow exposed to high quantities possibility arises that acts a compound via an autacoid effect. Proteins incorporated into matrix, like procollagen type I c‐peptide, augment after exposure. Melatonin augments osteoprotegerin, osteoblastic protein inhibits differentiation osteoclasts. Osteoclasts for as they degrade partly generating free radicals. Osteoclast activity resorption impaired radical scavenger properties melatonin. The administration doses (less than 10 mg daily) commonly used clinical studies on effect bone. However, human equivalent allometrically derived from animal 1–1.5 mg/kg/day range 75 kg adult, dose rarely clinically. Circadian rhythms are molecular oscillations governed by transcriptional–translational feedback loops (TTFLs) operating in nearly all cell types and fundamental to physiological homeostasis. Key circadian regulators, such as locomotor output cycles kaput (CLOCK), brain muscle ARNT-like 1 (BMAL1), period (PER), cryptochrome (CRY) gene families, regulate intracellular metabolism, oxidative balance, mitochondrial function, synaptic plasticity. disruption is known a central contributor the pathophysiology of neurodegenerative disorders. Disease-specific disruptions clock expression melatoninergic signaling potential early-stage biomarkers. Melatonin, neurohormone secreted pineal gland, modulates expression, stability, inflammatory responses. ABSTRACT Melatonin is a versatile neurohormone with diverse molecular functions, including sleep regulation, inflammation reduction, antioxidant activity, immune modulation, and anticancer properties. In bone metabolism, it promotes osteoblast formation, inhibits osteoclast synchronizes skeletal tissue rhythms to support health. As melatonin not yet clinically used for osteoporosis concerns about the current treatments' side effects remain, this review highlights its role in modulating interactions, particularly through regulation of receptor activator nuclear factor‐κB ligand osteoprotegerin, achieve bone‐forming antiresorptive effects. These have been demonstrated across various concentrations cell types Vivo models. Furthermore, safeguards microenvironment by mitigating oxidative stress inflammation, protecting osteoblasts, preventing loss, maintaining gut microbiota brain–gut–bone axis. attributes underscore melatonin's potential as an effective alternative or complementary therapy promoting health managing osteoporosis. Diabetes mellitus (DM) and its complications are one of the leading causes death worldwide, with approximately 700 million people expected to suffer from DM by 2045, according International Federation. is a progressive, chronic disease that affects various body systems can lead serious physical health complications. Type 2 diabetes (T2DM) has been shown be associated higher incidence sleep disturbances, which may due itself or secondary comorbidities DM. However, epidemiological evidence suggests risk obesity, metabolic syndrome, T2DM in individuals shorter duration poor quality sleep. Therefore, an assessment sleep, disorders their pharmacological correction recommended as comprehensive medical examination for existing groups at development. Among pharmacotherapeutic agents treatment disorders, melatonin (N-acetyl-5-methoxytryptamine) attracts special attention.
