Background Delivering prescription digital therapeutics (ie, evidence-based interventions designed to treat, manage, or prevent disorders via websites smartphone apps) in primary care could increase patient access substance use disorder (SUD) treatments. However, the optimal approach implementing remains unknown. Objective This pilot study is a precursor larger trial test whether implementation strategies (practice facilitation [PF] and health coaching [HC]) improve delivery of for SUDs care. mixed methods describes outcomes among patients 2 clinics presents qualitative findings on implementation. Methods From February 10 August 6, 2021, total 3 mental specialists embedded practices same integrated system were tasked with offering app-based SUD. In first half pilot, activities included training supportive tools. PF (at 1 clinic) HC clinics) added second half. All analyses relied secondary data, including electronic records therapeutic vendor data. Primary proportion reached by fidelity related ideal use. We used assess adherence planned barriers facilitators therapeutics. Results Of all 18 prescribed apps, (56%) downloaded app activated their prescription, 8 (44%) completed at least module content. Patients who per week average. Ideal (fidelity) was defined as completing 4 modules having monthly SUD-related visit; (6%) met these criteria weeks (of 12-week period). A 5 (28%) had prescriptions while available, (11%) successfully contacted, both declined coaching. Clinicians reported competing clinical priorities, technical challenges, logistically complex workflows part because apps required prescription. Some impacted staff turnover that coincided COVID-19 pandemic. The high engagement perception meet needs. Conclusions encountered real-world setting, especially staffing shortages, turnover, priorities clinic teams. randomized will clarify extent which Trial Registration ClinicalTrials.gov NCT04907045; https://clinicaltrials.gov/study/NCT04907045

Digital information technologies (DITs) can contribute to optimizing the quality and efficiency of healthcare delivery. However, profiles awareness use behavior DITs among Chinese nursing professionals remained limited. This study aimed investigate perceived acceptance, intention identify influencing factors in hospitals Shanghai. A total 1421 from 20 across Shanghai were selected as participants between August October 2021. After excluding missing values, 1395 included analyses. Using technology acceptance model, general was measured ease (PEU) usefulness (PU). Intention using two single 5-point Likert scales. Linear logistic regression models mediation analyses developed examine factors. All PU PEU items received affirmative responses (agree or strongly agree) over 50% participants. Of all participants, 1101 (78.9%) expressed DITs; 626 (44.9%) frequent users. Age, bachelor's degree, in-house training on DITs, school-based training, out-of-hospital associated with acceptance. Licensed practical nurse, deputy chief working years, significant predictors use. Vocational college diploma, tertiary level 1 2 hospitals, specialized mediated 42.6% (95%CI: 10.3% ~ 60.4%) effects DITs. suggests that although have positive strong they rarely their practice. Therefore, policies interventions should be enhance integration into professionals' daily

Abstract Background The integration of digital therapeutics (DTx) into the German statutory healthcare system marks a significant shift in medical practice through introduction innovative, reimbursable interventions for patient care. While DTx can bridge therapy gaps and enhance care, number prescriptions is increasing slowly. This study aims to explore challenges physicians face when integrating their treatment options changes this entails workflows. Methods A qualitative approach was adopted, gathering data from semi-structured interviews with 46 across Germany. Participants, sampled diversity specialty, experience, region, were interviewed between June 2022 August 2024. explored physicians’ knowledge, experiences, perspectives on DTx. Data analyzed using content analysis, combining deductive inductive coding. Results workflows impacts multiple levels. First, must become aware new regulations therapeutic possibilities, requiring information intake time feel adequately prepared prescribing Second, add complexity assessment, as evaluate factors such literacy, motivation, cognitive conditions, necessitating deeper understanding patients. Third, reshapes physician‒patient relationship, it alters interaction dynamics, redistributes responsibilities, poses communication challenges. Fourth, expand options, particularly by bridging delays enabling more personalized Finally, has potential drive long-term mindsets, fostering alignment multimodal patient-centered approaches. Conclusions offer opportunities care but also pose challenges, development skills or familiarization regulations, that necessitate adjustments Physicians are generally confronted an innovation demands time, reliable effectiveness, practical openness adopting health tools complementary options. However, reception vary among physicians, reflecting individual preferences Further research should these diverse adoption strategies examine effects delivery outcomes.

Digital health interventions have enormous potential to support patients and the public in achieving their goals. Nonetheless, many digital are failing effectively engage public. One solution that has been proposed is directly involve design process of these interventions. Although there consensus involving collaborative valuable, teams little guidance on how maximize value work. The main objective this study was understand patient involvement can be maximized, from perspective leaders patient-public partners. Using a qualitative descriptive methodology, we conducted semi-structured interviews with 19 9 Interviewees agreed however, they questioned if current methods were optimized ensure maximal value. suggested add through four different mechanisms: (1) by allowing an empowering intervention itself, (2) ensuring will engaging for users, (3) seamlessly implemented practice, (4) collaborations extend beyond initial product design. Overall, interviewees emphasized although historically focused improving crucial insights implementation planning as well used its own intervention. results paper provide clarity about ways made more valuable. use intentional approaches.

Background Violence against children (VAC) is a serious public health concern with long-lasting adverse effects. Evidence-based parenting programs are one effective means to prevent VAC; however, these interventions not scalable in their typical in-person group format, especially low- and middle-income countries where the need greatest. While digital delivery, including via chatbots, offers cost-effective scale up within settings, it crucial understand key pillars of user engagement ensure implementation. Objective This study aims investigate most combination external components optimize ParentText, an open-source chatbot-led intervention VAC Mpumalanga, South Africa. Methods will use mixed methods design incorporating 2 × factorial cluster-randomized controlled trial qualitative interviews. Parents adolescent girls (32 clusters, 120 participants [60 parents 60 aged 10 17 years] per cluster; N=3840 total participants) be recruited from Ehlanzeni Nkangala districts Mpumalanga. Clusters randomly assigned receive 1 4 packages that include ParentText alone or combined sessions facilitated WhatsApp support group. Quantitative data collected pretest-posttest parent- adolescent-reported surveys, facilitator-reported implementation data, digitally tracked data. Qualitative facilitators through over-the-phone individual semistructured interviews used expand interpretation understanding quantitative findings. Results Recruitment collection started August 2023 were finalized November 2023. The number enrolled 1009, 744 caregivers having completed onboarding intervention. Abstract Technology is playing an increasing role in healthcare, especially mental health. Traditional whether in-person or via telehealth, cannot by itself address the massive need for services. Standalone technology such as smartphone apps, while easily accessible, have seen limited engagement and efficacy on their own. Hybrid care – combination of synchronous telehealth appointments with use asynchronous digital tools applications, wearable devices, therapeutics has potential to offer best both worlds, providing increased access higher efficacy. In this paper, we present a framework highlighting key components hybrid models: intervention, human support, target population. This can be used evaluate existing models literature practice, identify areas opportunity, serve blueprint elements consider when designing new models.
