Abstract Intravoxel incoherent motion (IVIM) MRI is a non-contrast technique that estimates diffusion and perfusion parameters using multiple b-values. This systematic review meta-analysis aimed to compare the diagnostic accuracy of IVIM with contrast-enhanced (CE) in oncology imaging. Following PRISMA guidelines, comprehensive literature search across five databases identified studies compared CE patients brain, breast, rectal cancers. Meta-analyses were performed R software. A total 18 met inclusion criteria, encompassing 123 gliomas, 231 208 cancer lesions. demonstrated comparable performance dynamic susceptibility contrast (DSC) pooled AUC 0.84, sensitivity 92.27%, specificity 74.06%. In breast cancer, outperformed (DCE) MRI, AUCs 0.856 0.786. For DCE both showed moderate accuracy, 0.62 0.64, respectively. Correlation analysis relationships between DSC/DCE parameters, particularly gliomas cancer. presents promising alternative techniques, especially where it matched or exceeded performance. Further are needed validate IVIM’s efficacy more types strengthen its correlation parameters.

Abstract Digital subtraction angiography (DSA) is the gold standard for assessing cerebrovascular hemodynamics. DSA predominantly utilized to evaluate hemodynamic information of various cerebral diseases. However, relatively invasive and involves radiation exposure risks allergic reactions or renal dysfunction related iodine-based contrast agents. Magnetic resonance (MRDSA) with a gadolinium-based agent popular method hemodynamics without exposure. it has disadvantage risk allergy due lower temporal spatial resolution than DSA. In recent years, arterial spin labeling-based noncontrast MRDSA (ASL-MRDSA), which utilizes blood labeled radiofrequency pulses as an endogenous tracer, been developed. ASL-MRDSA exhibits same DSA; therefore, can yield dynamic about Pulsed ASL used major labeling method. development pseudocontinuous enabled high signal maintenance even at delayed phases. This advancement allows more detailed assessment vascular diseases, including artery severe stenosis, occlusion, moyamoya disease. Furthermore, combining superselective also complex malformations such addition, using ultrashort-echo time acquire signals before phase dispersion occurs, reduces artifacts induced by metallic materials flow. technique useful post-clipping, post-coiling, pseudostenosis. wheel scan mode full sampling achieves higher DSA, enables flow evaluation within giant aneurysms. Herein, we introduce basic principles novel techniques demonstrate its clinical applications, pitfalls, limitations in disease diagnoses.

Abstract Axial spondyloarthritis (axSpA) is a chronic inflammatory disease primarily affecting the axial skeleton, historically termed “human leukocyte antigen B27 positive spondyloarthropathy” or ankylosing spondylitis. Improved understanding of underlying pathologic mechanisms has led to an ongoing redefinition this state, recognition substantial phenotypic and biological variation, emphasis on earlier detection. Magnetic resonance imaging become central identifying confirming early axSpA, critical fulfilling promise slowing progression preventing irreversible structural abnormalities. Gadolinium-based contrast agents (GBCAs), essential for MRI, are facing renewed scrutiny due to gadolinium (Gd) retention and emerging toxicity profiles. While the link between less stable Nephrogenic Systemic Fibrosis (NSF) in renal impairment is established, deposition also observed brain, bone, skin across all GBCA classes, even patients with normal function. This finding has raised concerns led controversial concept of Gadolinium Deposition Disease (GDD). The present review synthesizes current evidence on clinical manifestations underlying mechanisms. It highlights pathways beyond traditional transmetallation, particularly endogenous nanoparticle formation as a key mechanism Gd release retention, potentially challenging stability assumptions macrocyclic agents. Structural factors (linear/macrocyclic; ionic/non-ionic) parameters (thermodynamic log K; kinetic kobs) influencing risk evaluated alongside regulatory responses. GBCAs should be viewed not inert diagnostics but complex, cumulative biological interactions. Future research focus developing non-gadolinium alternatives, validating biomarkers early detection conducting controlled trials chelation therapy efficacy. Clinicians must balance diagnostic benefits potential long-term risks, ensuring informed patient consent judicious use.

ABSTRACT The low proton density and high signal decay rate of pulmonary tissue have previously hampered the application magnetic resonance imaging (MRI) in clinical evaluation lung disorders. With continuing technical advances scanners, coils, pulse sequences, image postprocessing, MRI can provide structural functional information with faster speed improved quality, which has shown potential to be an alternative complementary diagnostic method chest computed tomography (CT). Compared CT, does not involve ionizing radiation, making it particularly suitable for pediatric patients, pregnant women, individuals requiring longitudinal monitoring. This narrative review focuses on recent techniques applications diseases, including parenchymal vascular diseases. Future developments, artificial intelligence‐driven technological optimization assisted diagnosis, hardware advancements, biomarkers validation, hold further enhance utility MRI.
