Background: Due to the complexity of chronic kidney disease (CKD) pathophysiology, biomarkers representing different mechanistic pathways have been targeted for study and development novel biomarkers. The discovery clinically useful CKD would allow identification those children at highest risk function decline timely interventions enrollment in clinical trials. Summary: Glomerular filtration rate proteinuria are traditional classify prognosticate progression practice but several limitations. Over recent decades, identified from blood or urine with metabolomic screening studies, proteomic an improved knowledge pathophysiology. This review highlights promising associated that could potentially serve as future prognostic markers CKD. Key Messages: Further studies needed validate putative biomarkers, particularly candidate proteins metabolites, improving management.

Metabolomics is defined as the quantification of small molecules (usually <1500 Daltons) within a biologic specimen ( e.g., plasma) ([1][1]). Detectable metabolites belong to various chemical classes like amino acids or lipids. Their concentrations are result interaction between genes,

Pediatric renal diseases encompass a diverse array of pathological conditions, often engendering enduring ramifications. Metabolomics, an emergent branch omics sciences, endeavors to holistically delineate alterations in metabolite compositions through the amalgamation sophisticated analytical chemistry techniques and robust statistical methodologies. Recent advancements metabolomics research within realm pediatric nephrology have been substantial, offering promising avenues for identification biomarkers, elaboration novel therapeutic targets, intricate elucidation molecular mechanisms. The present discourse aims critically review progress profiling pertinent disorders over previous 12 years.

ABSTRACT Background There is interest in identifying novel filtration markers that lead to more accurate GFR estimates than current (creatinine and cystatin C) are consistent across demographic groups. We hypothesize large-scale metabolomics can identify serum metabolites strongly influenced by glomerular rate (GFR) variables creatinine, which would be promising for future investigation. Methods evaluated the consistency of associations between measured (mGFR) 887 common, known quantified an untargeted chromatography- spectroscopy-based platform (Metabolon) performed on frozen blood samples from 580 participants Chronic Kidney Disease Children (CKiD), 674 Modification Diet Renal (MDRD) Study 962 African American Hypertension (AASK). metabolite–mGFR correlation association with metabolite class, molecular weight, assay measurement coefficient variation (CV). Among strong negative correlations mGFR (r < −0.5), we assessed additional age (height children), sex, race body mass index (BMI). Results A total 561 (63%) were negatively correlated mGFR. Correlations highly study, BMI categories (correlation 0.88 0.95). Amino acids, carbohydrates nucleotides often compared lipids, but there was no liquid chromatography/mass spectrometry CV. 114 27 consistently not associated sex or race. Conclusions The majority race, study. Metabolites non-association may represent candidate improve estimation GFR.
