Currently, infertility affects 8–12% of reproductive age couples worldwide, a problem that also women suffering from recurrent implantation failure (RIF). RIF is complex condition resulting many physiological and molecular mechanisms involving dynamic endometrium–blastocyst interaction. The most important are the endometrial receptivity process, decidualization, trophoblast invasion, blastocyst nesting. Although exact multifactorial pathogenesis remains unclear, studies have suggested association between hormone level imbalance, disturbances angiogenic immunomodulatory factors, certain genetic polymorphisms, occurrence RIF. These were performed in quite small groups. Additionally, results inconsistent ethnicities. present review briefly summarizes importance factors involved development could serve as diagnostic determinants. Moreover, our constitute part new platform for discovery novel therapeutic solutions

Establishing a successful pregnancy depends on the endometrium and embryo. It is estimated that suboptimal endometrial receptivity account for one-third of implantation failures. Despite indepth understanding processes associated with embryo-endometrial cross-talk, little progress has been achieved diagnosis treatments receptivity. This retrospective study included women undergoing their first frozen-thawed embryo transfer (FET) cycles at our reproductive medicine center from March 2021 to August 2021. Transvaginal three-dimensional (3D) ultrasound was performed in morning day all thawed patients, evaluate receptivity, including thickness, echogenicity, volume, movement blood flow.A total number 562 patients FET 315 pregnancies (56.0%) analyzed. found only echo central line different between pregnant group non-pregnant group. Other parameters, such as peristalsis, or flow were not statistically two groups. Then, according relationship groups clinical rate, score 0 2 respectively scored. The sum scores six items patient's score. showed rate increased increased, when reaches least 5, significantly improved (63.7% versus 49.5%, P=0.001).We developed an scoring system demonstrated its validity. may aid clinicians choosing useful marker practice informing further research.

As a rate-limiting step in pregnancy, embryo implantation is highly dependent on intercellular communication. Extracellular vesicles (EVs) are newly identified to be important the course of EVs have been isolated from wide variety biofluids and tissues, including plasma, liver, uterine, semen, embryo, etc. The present future use not only as biomarkers, but also targeting drug delivery system, promisingly pave way for advanced comprehension failure reproductive diseases. However, precise mechanisms has elucidated yet. Herein, we summarize current knowledge diverse effects various sources their cargos such microRNA, long non-coding RNA, protein, implantation, potential diseases recurrent failure, polycystic ovary syndrome endometriosis. An abnormal endometrial microbiota has been suggested to impair the process of embryo implantation, thus leading repeated implantation failure (RIF) in women undergoing vitro fertilization (IVF). However, molecular mechanisms linking uterine and IVF out-comes are still an open question. The aim this cohort study was outline relationship between microbiota, inflammation outcomes. To purpose, selected components “cytokine network” were analyzed presenting RIF divided eubiosis dysbiosis groups, according percentage lactobacilli (≥90% or <90%, respectively). Dysbiosis group presented significantly higher tissue concentrations inflammatory markers (IL-6, IL-1β, HIF-1α COX-2) lower levels anti-inflammatory/well-being factors, IL-10 IGF-1, with respect eubiosis. Moreover, Lactobacillus negatively related molecules positively IL-10/IGF-1. Interestingly, number attempts directly factors COX-2, IL-1β group. Overall, demonstrated be associated inflammation-related changes affecting underlining importance assessing patients IVF.

Abstract Expressional profiling of the endometrium enables personalised timing window implantation (WOI). This study presents and evaluates a novel analytical pipeline based on TAC-seq (Targeted Allele Counting by sequencing) method for endometrial dating. The expressional profiles were clustered, differential expression analysis was performed model development group, using 63 biopsies spanning over proliferative (PE, n = 18), early-secretory (ESE, mid-secretory (MSE, 17) late-secretory (LSE, 10) phases natural cycle. A quantitative predictor trained group validated sequenced samples from healthy women, consisting 52 paired taken ESE MSE five LSE phase 31 individuals. Finally, developed test applied to 44 patients diagnosed with recurrent failure (RIF). In validation (n 57), we detected displaced WOI in 1.8% fertile women. RIF significantly higher proportion shifted than set 15.9% (p 0.012), respectively. evaluated an average cross-validation accuracy 98.8% 98.2% group. beREADY screening sensitive dynamic detection selected transcriptome biomarkers, providing accurate prediction receptivity status.

After more than four decades of assisted reproductive technology (ART) practice worldwide, today 60% women undergoing in vitro fertilization (IVF) treatments fail to become pregnant after the first embryo transfer and nearly 20% patients are suffering from unexplained recurrent implantation failures (RIFs) repeated pregnancy loss (RPL). The literature reported different causes RIF–RPL, mainly multifactorial, endometrial idiopathic. RIF remains a black box because complicated categorization this physio-pathological dysregulation process ovarian stimulation. Many options were suggested as solutions treat RIF–RPL with controversial results on their usefulness. In article, we reviewed possible therapeutic improve rates clinical outcomes. Based our experience believe that endometrium immunomodulation intrauterine insemination activated autologous peripheral blood mononuclear cells (PBMCs) or platelet-rich plasma (PRP) can be promising solution. On other hand, lymphocyte balance typing, specific cytokines interleukins profiling proposed predictive biomarkers before transfer.
