Changing lifestyles, over-nutrition, and physical inactivity have promoted its development. MASLD is typically accompanied by obesity strongly linked to metabolic syndromes. Given that prevalence on the rise, there an urgent need elucidate pathogenesis. Abstract Background: Hepatic fibrosis (HF) is a critical pathological process in chronic liver diseases, and its progression closely associated with gut microbiota dysbiosis circadian rhythm disruption. However, the interplay between these factors HF remains poorly understood. This study aimed to investigate relationship composition, disturbances, HF, providing new insights into potential therapeutic strategies. Methods: A cross-sectional was conducted, enrolling patients healthy controls. Liver stiffness measurement (LSM) assessed using transient elastography. Circadian status evaluated Morningness-Eveningness Questionnaire-5 (MEQ-5). Gut composition analyzed via 16S rRNA sequencing, differences microbial diversity taxa abundance were compared groups. Correlation analyses performed explore associations microbiota, LSM, rhythm. Results: Patients exhibited significant alterations at both phylum genus levels (p<0.05, r = − 0.244). The relative abundances of Escherichia-Shigella, Klebsiella, Pseudomonadota, Ruminococcus gnavus group, Enterocloster significantly increased, while Dorea, Holdemanella, [Ruminococcus] gauvreauii group, [Eubacterium] ventriosum CAG-352, Marvinbryantia markedly decreased. These shifts enhanced intestinal inflammation hepatic immune activation. Notably, Escherichia-Shigella may contribute LPS-TLR4/inflammasome activation, inflammatory cytokine release, reduced short-chain fatty acid (SCFA) production. Conversely, SCFA-producing bacteria Firmicutes showed protective role by mitigating lipid accumulation. Furthermore, disruption negatively correlated an increased Mediterraneibacter observed disturbances. To explore the correlation of dietary index for gut microbiota (DI-GM) with non-alcoholic fatty liver disease (NAFLD). Data 6,711 participants were extracted from National Health and Nutrition Examination Survey (NHANES) during 2007-2018. A weighted logistic regression analysis was employed assessment DI-GM NAFLD, a restricted cubic spline (RCS) implemented to examine potential non-linear associations.
