Alzheimer's disease (AD) is an aging-related irreversible neurodegenerative affecting mostly the elderly population. The main pathological features of AD are extracellular Aβ plaques generated by APP cleavage through amyloidogenic pathway, intracellular neurofibrillary tangles (NFT) resulting from hyperphosphorylated tau proteins, and cholinergic neurodegeneration. However, actual causes unknown, but several studies suggest hereditary mutations in PSEN1 -2, APOE4, APP, TAU genes major perpetrators. In order to understand etiology pathogenesis AD, various hypotheses proposed. These include following hypotheses: amyloid accumulation, tauopathy, inflammation, oxidative stress, mitochondrial dysfunction, glutamate/excitotoxicity, deficiency, gut dysbiosis. Currently approved therapeutic interventions donepezil, galantamine, rivastigmine, which cholinesterase inhibitors (ChEIs), memantine, N-methyl-d-aspartate (NMDA) antagonist. treatment strategies focus on only symptomatic management attenuating symptoms not regeneration neurons or clearance Tau. This review focuses pathophysiology, novel targets, disease-altering treatments such as α-secretase modulators, active immunotherapy, passive natural antioxidant products, nanomaterials, antiamyloid therapy, aggregation inhibitors, transplantation fecal microbiota stem cells, microtubule stabilizers that clinical trials still under investigation.

Significant changes in gut microbial composition are associated with chronic liver disease. Using preclinical models, it has been demonstrated that ethanol/alcohol-induced disease is transmissible through fecal microbiota transplantation (FMT). So, the survival rate of people severe alcoholic hepatitis got better, which suggests makeup and function play a role metabolic The leaky intestinal barrier plays major influencing metabolic-related development microbiota. As result, viable bacteria products can be transported to liver, causing inflammation, contributing hepatocyte death, fibrotic response. starts gets worse, dysbiosis linked immune system, bile acid composition, gut. Metabolic-related disease, as well its self-perpetuation, will using data from human studies. Further, we summarize how untargeted treatment approaches affect including dietary changes, probiotics, antibiotics, FMT. It discusses targeted therapies improve various areas. These may options.

Post-stroke depression (PSD) is one of the most common and devastating neuropsychiatric complications in stroke patients, affecting more than one-third survivors ischemic (IS). Despite its high incidence, PSD often overlooked or undertreated clinical practice, effective preventive measures therapeutic interventions remain limited. Although exact mechanisms are not fully understood, emerging evidence suggests that gut microbiota plays a key role regulating gut-brain communication. This has sparked great interest relationship between microbiota-gut-brain axis (MGBA) PSD, especially context cerebral ischemia. In addition to microbiota, another important factor barrier, which acts as frontline sensor distinguishing beneficial harmful microbes, inflammatory responses immunomodulation. Based on this, this paper proposes new approach, microbiota-immune-barrier axis, only closely related pathophysiology IS but may also play critical occurrence progression PSD. review aims systematically analyze how affects integrity function barrier after through immunomodulation, leading production exacerbation depressive symptoms addition, we will explore existing technologies can assess MGBA potential strategies for with hope providing insights future research interventions.

Stroke represents a predominant cause of mortality and disability on global scale, impacting millions annually exerting considerable strain healthcare systems. The incidence stroke exhibits regional variability, with ischemic accounting for the majority occurrences. Post-stroke complications, such as cognitive impairment, motor dysfunction, recurrent stroke, profoundly affect patients’ quality life. Recent advancements have elucidated microbiota-gut-brain axis (MGBA), underscoring complex interplay between gut health brain function. Dysbiosis, characterized by an imbalance in microbiota, is significantly linked to elevated risk unfavorable outcomes. MGBA plays crucial role modulating immune function, neurotransmitter levels, metabolic byproducts, which may intensify neuroinflammation impair cerebral health. This review elucidates pathophysiology explores potential gut-targeted therapeutic strategies reduce promote recovery, including probiotics, prebiotics, pharmacological interventions, dietary modifications. However, current prevention treatment based intestinal flora still face many problems, large difference individual flora, stability efficacy, long-term safety need be considered. Further research needs strengthened its better application clinical practice.
