Objective: Postoperative delirium (POD) commonly occurs in elderly individuals following hip fracture surgery, with unclear pathophysiological mechanism. Inflammation is a known factor affecting the onset of delirium. The current work aimed to examine associations preoperative immune inflammation-related indicators POD occurrence cases surgery. Methods: retrospective cohort study included 437 administered surgery from January 2018 December 2023. clinicodemographic data and laboratory findings all were retrospectively analyzed. Immune assessed, eg, MLR, NLR PLR, as well SII SIRI. bootstrap method was employed assign at 7:3 training (48 258 no-POD groups, respectively) internal validation (13 118 cohorts. Next, LASSO, univariable multivariable logistic regression analyses applied determine risk factors cohort, based on which nomogram model built. obtained examined for accuracy by calibration plot analysis. Finally, nomogram's clinical value assessed decision curve analysis (DCA), followed cohort. Results: Of cases, 61 developed POD, indicating incidence 13.96%. LASSO revealed SIRI independently predicted had an area under (AUC) 0.991 (95% CI 0.983~0.998) versus 0.986 0.966~1.000) Calibration nomogram-predicted actual probabilities line. DCA demonstrated novel could confer net benefits prediction Conclusion: predict Keywords: postoperative delirium, fracture, elderly, inflammation related indicators,
