Microbial contamination in the hospital environment is a major concern for public health, since it significantly contributes to onset of healthcare-associated infections (HAIs), which are further complicated by alarming level antimicrobial resistance (AMR) HAI-associated pathogens. Chemical disinfection control bioburden has temporary effect and can favor selection resistant pathogens, as observed during COVID-19 pandemic. Instead, probiotic-based sanitation (probiotic cleaning hygiene system, PCHS) was reported stably abate AMR, HAIs. PCHS action not rapid nor specific, being based on competitive exclusion, but addition lytic bacteriophages that quickly specifically kill selected bacteria shown improve effectiveness. This study aimed investigate such combined probiotic-phage (PCHSφ) two Italian hospitals, targeting staphylococcal contamination. The results showed PCHSφ could provide higher removal staphylococci, including strains, compared with disinfectants (-76%, p < 0.05) alone (-50%, 0.05). Extraordinary sporadic chlorine appeared compatible PCHSφ, while frequent routine usage inactivated probiotic/phage components, preventing action. collected data highlight potential biological better infectious risk healthcare facilities, without worsening pollution AMR concerns.

Carbapenem resistance has become major concern in healthcare settings globally; therefore, its monitoring is crucial for intervention efforts to halt spread. During May 2019-April 2022, 2170 clinical strains were characterized antimicrobial susceptibility, genes, replicon and sequence types. Overall, 42.1% isolates carbapenem-resistant, significantly associated with Klebsiella pneumoniae (K. pneumoniae) (p = 0.008) Proteus species 0.043). Plasmid types IncFII, IncA/C, IncN, IncL/M, IncFIIA IncFIIK observed. The hospital environment represents an important mediator for the transmission of healthcare-associated infections through direct and indirect hand contact with hard surfaces textiles.
