Multiple sclerosis (MS) is a demyelinating neurological disorder with highly heterogeneous clinical presentation and course of progression. Disease-modifying therapies are the only available treatment, as there no known cure for disease. Careful selection suitable necessary, they can be accompanied by serious risks adverse effects such infection. Magnetic resonance imaging (MRI) plays central role in diagnosis management MS, though MRI lesions have displayed moderate associations MS outcomes, clinico-radiological paradox. With advent machine learning (ML) healthcare, predictive power improved leveraging both traditional advanced ML algorithms capable analyzing increasingly complex patterns within neuroimaging data. The purpose this review was to examine application MRI-based prediction disease Studies were divided into five main categories: predicting conversion clinically isolated syndrome cognitive outcome, EDSS-related disability, motor disability activity. performance models discussed along highlighting influential MRI-derived biomarkers. Overall, presents promising avenue prognosis. However, integration biomarkers other multimodal patient data shows great potential advancing personalized healthcare approaches MS.

Neuroinflammation is involved in the pathophysiology of Alzheimer's disease (AD), including immune-linked genetic variants and molecular pathways, microglia astrocytes. Multiple Sclerosis (MS) a chronic, immune-mediated with environmental risk factors neuropathological features. There are clinical pathobiological similarities between AD MS. Here, we investigated shared susceptibility MS to identify putative pathological mechanisms neurodegeneration immune system. We analysed GWAS data for late-onset (N cases = 64,549, N controls 634,442) 14,802, 26,703). Gaussian causal mixture modelling (MiXeR) was applied characterise architecture overlap Local correlation Analysis [co]Variant Association (LAVA). The conjunctional false discovery rate (conjFDR) framework used specific loci, which functional annotation conducted FUMA Open Targets. MiXeR analysis showed comparable polygenicities (approximately 1800 trait-influencing variants) 20% despite negligible (rg 0.03), suggesting mixed directions effects across variants. conjFDR identified 16 8 having concordant direction Annotated genes loci were enriched signalling pathways inflammation structural organisation neurons. Despite low global correlation, current results provide evidence polygenic neurodegeneration, highlighting new opportunities future investigation.

Abstract The ongoing developments of psychiatric classification systems have largely improved reliability diagnosis, including that schizophrenia. However, with an unknown pathophysiology and lacking biomarkers, its validity still remains low, requiring further advancements. Research has helped establish multiple sclerosis (MS) as the central nervous system (CNS) disorder established pathophysiology, defined biomarkers therefore good significantly treatment options. Before proposing next steps in research aim to improve diagnostic process schizophrenia, it is imperative recognize clinical heterogeneity. Indeed, individuals schizophrenia show high interindividual variability terms symptomatic manifestation, response treatment, course illness functional outcomes. There also a multiplicity risk factors contribute development Moreover, accumulating evidence indicates several dimensions psychopathology cross current categorizations. Schizophrenia shares number similarities MS, which demyelinating disease CNS. These appear context age onset, geographical distribution, involvement immune-inflammatory processes, neurocognitive impairment various trajectories course. This article provides critical appraisal taking into consideration advancements been made diagnosis management MS. Based on comparison between two disorders, key directions for studies are formulated. All them converge necessity deconstruct psychosis spectrum adopt dimensional approaches deep phenotyping refine boundaries.

Background: To reduce the risk of long-term disability in people with Multiple Sclerosis (pwMS), an increasing number diseasemodifying immune therapies (DMT) are available, involving diverse mechanisms action, levels efficacy, treatment risks, and tolerability aspects.Including patient preferences expectations shared decision-making may improve satisfaction, adherence, persistence.Purpose: investigate alignment individual pwMS their actual DMT its effect on health-related quality life (HRQoL), discontinuation. Methods: A total 401 beginning a new were enrolled from 2015 to 2018 non-interventional study at three German MS centres.Patient regarding DMT, TSQM-9, SF36, self-reported adherence as well relapses EDSS recorded baseline every 3 6 months for up years. Results: Efficacy highest-ranking baseline.Actual selection corresponded more closely safety than tolerability, or convenience preferences.Participants reported excellent throughout study.DMT persistence was 69.0%, earlier discontinuation injectable vs oral infusion therapies.Breakthrough disease, rather patient-reported outcomes, main driver discontinuation. For all routes administration, global satisfaction increased over time despite lower convenience.Several patterns changing observed. Conclusion:This provides insight into interaction experience.Treatment decisions should be aligned promote continuous adherence.

Multiple sclerosis (MS) is the most common cause of non-traumatic long-term disability in young adults. Whole-body cryostimulation (WBC) a cold-based physical therapy known to induce physiological exercise-mimicking changes cardiovascular, neuromuscular, immune, and endocrine systems influence functional psychological parameters by exposing human body cryogenic temperatures (≤−110 °C) for 2–3 min. The purpose this scoping review present an overall view on potential role WBC as adjuvant treatment MS. Abstract Objective To estimate the current nationwide prevalence of falls, injurious concerns about falling, and information on fall prevention among people with multiple sclerosis (PwMS). Design This is a cross-sectional national web-based survey that included 965 adult PwMS. Participants self-reported falls experienced in past 6 months. also provided their falling received. Results A total 56% reported The at population level ranges between 53% 59%. Most occur inside participants’ homes (68%). About 30% fallers an fall. respondents, 87% expressed being concerned 68% they had cut down activities due to falling. Among participants who received (64%), only 9% formal course. Conclusion Despite advances research over last decades, continues be highly prevalent problem for one-third those result injuries. Concerns non-fallers affect performance daily independence. Few receive education or training.

Some of the most prevalent neurodegenerative disorders, including Alzheimer’s and Parkinson’s disease, are proteinopathies characterized by accumulation specific protein aggregates in brain. Such misfolded can trigger modulation innate adaptive immune systems subsequently lead to chronic neuroinflammation that drives onset progression diseases. Since there is still no effective disease-modifying treatment, new therapeutic targets for have been sought. The endocannabinoid system, particular cannabinoid CB2 receptors, extensively studied, due their important role neuroinflammation, especially microglial cells. Several studies shown promising effects receptor activation on reducing aggregation-based pathology as well attenuating inflammation several dementia-related symptoms. In this review, we discuss available data receptors potential benefits limitations agonists these therapy proteinopathies.
