Vagus nerve stimulation (VNS) is a technology that provides electrical to the cervical vagus and can be applied in treatment of wide variety neuropsychiatric systemic diseases. VNS exerts its effect by stimulating vagal afferent efferent fibers, which project upward brainstem nuclei relayed circuits downward internal organs influence autonomic, neuroendocrine, neuroimmunology systems. The neuroimmunomodulation mediated through cholinergic anti-inflammatory pathway regulates immune cells decreases pro-inflammatory cytokines. Traditional non-invasive have Food Drug Administration (FDA)-approved indications for patients with drug-refractory epilepsy, treatment-refractory major depressive disorders, headaches. number clinical trials translational studies explore therapeutic potentials mechanisms increasing. In this review, we first introduced anatomical physiological bases immunomodulating functions VNS. Vagus nerve stimulation (VNS) has gained enormous traction as a promising bioelectronic therapy. In particular, the delivery of VNS paired with training to promote neural changes demonstrated clinical success for stroke recovery and found far-reaching application in other domains, from autism psychiatric disorders normal learning. The been extensively documented. Here, we consider more unusual question: why does have such broad utility, perhaps importantly, when not work? We present discussion concepts that underlie therapy an anthology studies describe conditions which these are violated fails. focus specifically on mechanisms engaged by implanted VNS, how parameters stimulation, method, pharmacological manipulations, accompanying comorbidities, specifics concurrent interact impact efficacy As is increasing translated implementation, clear understanding it does, critically, work fundamental this approach.

Stroke poses a serious threat to human health and burdens both society the healthcare system. Standard rehabilitative therapies may not be effective in improving functions after stroke, so alternative strategies are needed. The FDA has approved vagus nerve stimulation (VNS) for treatment of epilepsy, migraines, depression. Recent studies have demonstrated that VNS can facilitate benefits rehabilitation interventions. coupled with upper limb enhances recovery function patients chronic stroke. However, its invasive nature limits clinical application. Researchers developed non-invasive method stimulate (non-invasive stimulation, nVNS). It been suggested nVNS could promising muscle stroke patients. In this article, we review current researches preclinical as well potential applications We summarize parameters, advantages, mechanisms, adverse effects applications, future challenges directions cerebral treatment. These indicate efficacy reducing volume attenuating neurological deficits ischemic models. While more basic research is required fully understand mechanisms efficacy, especially Phase III trials large number patients, these data suggest applied easily only possible secondary prophylactic but also adjunctive acute near future.

Paired vagus nerve stimulation (VNS) has emerged as a promising strategy to potentiate recovery after neurological injury. This approach, which combines short bursts of electrical the with rehabilitation exercises, received approval from US Food and Drug Aministration in 2021 first neuromodulation-based therapy for chronic stroke. Because this treatment is increasingly implemented clinical practice, there need take stock what we know about approach have yet learn. Here, provide survey on foundational basis VNS stroke offer insight into mechanisms that underlie potentiated recovery, focusing principles neuromodulatory reinforcement. We discuss current state observations regarding synaptic reorganization motor networks are enhanced by VNS, propose other prospective loci neuromodulation should be evaluated future. Finally, highlight future opportunities challenges faced translated use. Collectively, clearer understanding mechanistic may reveal ways maximize its benefits.

Stroke is a disease with high incidence and disability rate, resulting in changes neural network corticoid-subcortical excitability various functional disabilities. The aim of the present study was to discuss current status research limitations potential direction application noninvasive brain stimulation (NIBS) on post-stroke patients. This literature review focused clinical studies reviews. Literature retrieval conducted PubMed, Cochrane, Scopus, CNKI, using following keywords: Repeated transcranial magnetic stimulation, Transcranial direct alternating ultrasound, Noninvasive vagus nerve Stroke, Rehabilitation. We selected 200 relevant publications from 1985 2022. An overview recent use NIBS patients, including its mechanism, therapeutic parameters, effects, safety, presented. It found that has positive effects dysfunctions motor, sensory, cognitive, speech, swallowing, depression after stroke, but standardized stimulus programs are still lacking. suggests rTMS tDCS more beneficial while tFUS tVNS currently less studied for rehabilitation, also interventions.

Cerebrovascular accident (CVA) or stroke is a devastating neurological condition with dismal prognosis associated recurrent episodes that further damage the neuronal networks, thus disabling plasticity. Vagus nerve stimulation (VNS) has been used in clinical practice to treat epilepsy for several decades and well accepted as safe procedure devoid of serious adverse events. Bailey Bremer demonstrated VNS capabilities stimulate pathways enhance recovery damaged cerebral function. Further studies have strengthened these observations, while technology improved tolerability implants, resulting applications epilepsy. Several animal models on neural plasticity our understanding its ability provide neuromodulation improve patients. The closed-loop vagus individualized parameters combined physical therapy appears be an attractive option today. also being tested noninvasive trans-cutaneous modality patient acceptance tolerability.
