Chronic pain is highly prevalent in the pediatric population. Many factors are involved transition from acute to chronic pain. Currently, there conceptual models proposed, but they lack a mechanistically sound integrated theory considering stages of child development. Objective biomarkers critically needed for diagnosis, risk stratification, and prognosis pathological chronification. In this article, we summarize current evidence on mechanisms transitions infants children through developmental lens. The goal identify gaps outline future directions basic clinical research toward developmentally informed chronification At outset, importance objective outlined, followed by summary adults, order contrast with Evidence presented show that may have its origin insults early life, which prime development later life. ABSTRACT Objective Posttraumatic stress disorder (PTSD) and traumatic life events are often coupled to chronic pain, possibly linked by central sensitization. We wanted assess the prevalence of PTSD in pain patients a German university hospital outpatient clinic. Moreover, we evaluated extent indicators co-occurring traits sensitization comorbid patients. Methods retrospectively divided 914 into four groups depending on their trauma severity: no trauma, accidental interpersonal PTSD. collected electronic drawings focusing area widespreadness, as well information about intensity, sleep impairment, disability, stress, anxiety, depression, somatization. Differences between were calculated using Kruskal-Wallis with post-hoc Mann-Whitney tests. Results Of patients, 231 (25%) had 210 (23%) traumas, 283 (31%) 99 (11%) PTSD, 91 (10%) could not be classified. observed statistically significant differences maximal The severity symptoms increased severity. Conclusions Traumatic frequent negative impact sensory sensitivity (anxiety, somatization), compatible Therefore, heightened awareness comorbidity experiences is recommended.

Digital therapeutics (DTx, software as a medical device) provide personalized treatments for chronic diseases and expand precision medicine beyond pharmacogenomics-based pharmacotherapies. In this perspective article, we describe how DTx low back pain (CLBP) can be integrated with pharmaceutical drugs (e.g., NSAIDs, opioids), physical therapy (PT), cognitive behavioral (CBT), patient empowerment. An example of an FDA-authorized CLBP is RelieVRx, prescription virtual reality (VR) app that reduces severity adjunct treatment moderate to severe pain. RelieVRx immersive VR system delivers at-home management modalities, including relaxation, self-awareness, distraction, guided breathing, education. The mechanism action aligned recommendations from the American College Physicians use non-pharmacological modalities first-line CLBP. Herein, discuss multimodal options integrating conventional exposome-responsive, just-in-time adaptive interventions (JITAI). Given flexibility software-based therapies accommodate diverse digital content, also suggest music-induced analgesia increase clinical effectiveness offers opportunities simultaneously address crisis opioid epidemic while supporting patients healthcare providers improve outcomes.

Abstract Background and Objective Low back pain (LBP), in particular non‐specific low (NSLBP), which accounts for approximately 90% of LBP, is the leading cause years lived with disability worldwide. Childhood abuse has been linked to poorer health outcomes in individuals with fibromyalgia (FM), and this relationship may be affected by cognitive processes such as pain catastrophizing mindfulness. This study investigated the impact of childhood on pain-related symptoms functioning (FM) examined how critical psychosocial constructs mindfulness might moderate relationship. We hypothesized that would greater physical lower functioning, influencing these associations. The sample consisted 113 women FM. Correlational analyses were conducted examine between (pain severity, interference, impact, functioning). Moderation outcomes. As expected, was generally associated elevated symptomatology reduced higher levels impact. However, contrary our hypothesis, not directly or functioning. Pain significant moderators Specifically, related interference only at low high Our findings suggest while did patients, factors like play roles moderating effects. These results underscore importance assessing for psychological management fibromyalgia, especially patients a history abuse. Further research is needed explore relationships more diverse samples develop personalized targeted interventions patient population.

Childhood adversity can have a lasting negative impact throughout one's life. Youth with pain conditions consistently report higher rate of adverse childhood experiences (ACEs) when compared their healthy peers. Adolescents experiencing in more than 1 region tend to greater symptom burden and reduced quality Research on the association between ACEs multisite adolescents is sparse. The objective our study was investigate cumulative self-report early adolescence using data from Adolescent Brain Cognitive Development study. We used 19-region body map evaluate presence regional (1-2 regions) (≥3 regions). analyzed multinominal logistic regression, adjusting for sociodemographic factors including pubertal status, sex, race/ethnicity, income-to-needs ratio. included total 7582 children aged 12 13 years, 33.4%, 24.0%, 13.2%, 8.6% reporting 1, 2, 3, 4+ ACEs, respectively. Moreover, 30.7%, 24.2%, 15.2%, 10.1% reported Those (adjusted odds ratio 1.62, 95% confidence interval 1.24-2.12) 3 1.44, 1.14-1.82) were likely no ACEs. showed potential dose-response relationship pain, suggesting that particularly may emerge earlier previously documented.
