Background Severe self-harm leading to extensive hospitalization generates extreme challenges for patients, families, and health services. Controversies regarding diagnoses care often follow. Most evidence-based treatments targeting are designed borderline personality disorder (BPD). However, current knowledge about mental status among individuals with severe is limited. Objectives To investigate psychopathology patients extensively hospitalized due or frequent self-harming behaviors. Method A cross sectional study (period 2019–2021) psychiatric inpatients (>18 years) (>5) long (>4 weeks) admissions last year self-harm. The target sample ( N = 42, from 12 hospitals across all Norwegian regions) was compared admitted outpatient (PD) treatment within specialist services in the same period 389). Clinicians performed interviews on psychopathology, supplemented by self-report. Results were young adults, mainly female, considerable behaviors, both significantly more than comparison group. majority groups reported onset <18 years. increasing severity of acts suicidal intention over time. Both samples had high levels childhood trauma, impaired functioning, a fulfilled criteria PD. In sample, comorbid depression, PTSD, anxiety disorders, substance use occurred frequently 50%, psychosis/dissociative disorder/autism spectrum disorder/ADHD (outpatient sample: 9%). 35% screened cut-off possible intellectual disability. poor psychosocial functioning health-related quality life – greater impairment sample. Abstract In this chapter we consider identity: what William James (1890) labeled as “the most puzzling puzzle with which psychology has to deal.” The is comprised of three parts. First, introduce common understandings identity and disturbance in the context ICD-11 personality disorders (PD). particular, focus on temporal sense narrative identity, understood dynamic story people construct about their past, present, presumed future a promising, though less-studied aspect PD. Next, review state-of-the-art assessments (e.g., AIDA, LoPF-Q; STiP-5, AMPD) discuss clinical utility. We end by discussing role (narrative) PD psychotherapy, particular eye toward agency.

Introduction This study investigates age-related differences in Borderline Personality Disorder (BPD) traits and childhood maltreatment (CM) experiences among adolescents, young adults, older adults within a clinical sample. Methods A cross-sectional design was employed, involving 2029 outpatients aged 15-50 years from the Shanghai Mental Health Center. BPD were assessed using Diagnostic Questionnaire 4th Edition Plus (PDQ-4+), CM evaluated Child Trauma Short Form (CTQ-SF). Participants categorized into three age groups: adolescents (15-21 years), (22-30 (31-50 years). Results Adolescents reported significantly higher frequencies of diagnoses compared to ( p =0.036). Specifically, identity disturbance impulsivity more pronounced <0.001). Additionally, levels emotional F =15.987, <0.001) physical abuse =12.942, =0.002), while neglect. Logistic regression analysis identified key criteria subtypes that differentiated adults. Discussion The findings underscore importance age-specific interventions treating addressing maltreatment. exhibit distinct patterns experiences, necessitating tailored therapeutic approaches.

Background The majority of mental health services include patients with personality disorder (PD) and comorbid conditions. Alexithymia, a psychological construct referring to difficulties in identifying describing internal states, may represent challenge the psychotherapeutic treatment PD. This study aimed investigate prevalence alexithymia among specialized PD services, differences according severity type, longitudinal course during treatment. Methods included 1,019 treated units, 70% them above diagnostic threshold [borderline PD, 31%; avoidant 39%; not otherwise specified (PD-NOS), 15%; other PDs, more than one 24%]. Alexithymia was measured repeatedly throughout using Toronto Scale (TAS-20) self-report questionnaire. Supplementary outcomes global psychosocial function health-related life quality. Linear mixed models were applied for data analysis. Results highly prevalent sample: 53% subjects reported high levels 20% moderate levels. TAS-20 subscale Difficulty Identifying Feelings strongly associated borderline while Describing closely linked For all TAS subscales, poorer abilities severe higher anxiety depression, functioning Both measures improved significantly effect sizes regardless initial status. In total, 19% full remission alexithymia. Conclusion is common problem PDs dysfunction, rates varying across type severity. demonstrates improvement services. Further research should evaluate effectiveness different treatments interventions reducing patients.

Abstract With the dimensional shift, personality pathology is now commonly conceptualized using a combination of functioning and (pathological) traits. Personality has been deemed more sensitive to treatment than specific trait problems. To empirically examine just that, goal this pilot study was simultaneously compare changes in (LPFS-BF 2.0), pathological traits (PID-5-BF), normal-range (BFI-2) among individuals receiving integrative, dynamic-relational psychotherapy (baseline n = 52, follow-up 31) matched control group ( 31). The results showed that clients had stronger when compared group. In addition, lower on were inclined drop-out therapy. This points unique clinical utility provides foundation for future research focusing sensitivity within context psychotherapy.

Abstract Background Since its introduction in the diagnostic manuals DSM-5 and ICD-11, construct of personality functioning has gained increasing attention. However, it remains unclear which factors might predict improvement functioning. Methods We examined a sample 648 completed psychodynamic psychotherapies conducted by 172 therapists at Heidelberg Institute for Psychotherapy. A machine learning approach was used to filter variables that are relevant prediction from broad data set collected beginning each psychotherapy. Results On average, we found an 0.24 (SD = 0.48) OPD-SQ. This corresponds medium effect Patients with initially high impairment experienced particularly large improvements. Overall, number proved be predictive Limitations social activity due physical emotional problems one most important predictors improvement. Most sizes were small. Conclusions during psychotherapy is determined more sum small effects than individual variables. In particular, capture areas life robust predictors.
