Introduction The mesial temporal lobe plays a distinct role in epileptogenesis, and tumors this part of the brain potentially have specific clinical radiological features. Differentiating high-grade from lower-grade or non-neoplastic lesions can be challenging, preventing decision for early resection that critical tumors. Methods A tumor database was analyzed retrospectively to identify patients with temporomesial We determined features (age, sex, symptoms leading presentation) as well neuroradiological (tumor location presence contrast enhancement on initial magnetic resonance imaging (MRI)) neuropathological findings. Results identified 324 39 involved lobe. 77% occured males, presented seizures, regardless type grade. In 50 years older, 90% were male 80% had glioblastoma (GBM); there no GBM younger than years. 50% GBMs lacked enhancement. Male sex significantly associated GBM. both contrast-enhancing non-enhancing tumors, age older also Conclusion middle-aged tumor, is most likely diagnosis even when MRI Prolonged diagnostic workup surveillance strategies should avoided may justified these patients.

Glioblastoma (GBM) is a highly malignant, invasive, and poorly prognosed brain tumor. Unfortunately, active comprehensive treatment does not significantly prolong patient survival. With the deepening of research, it has been found that gut microbiota plays certain role in GBM, can directly or indirectly affect efficacy immune checkpoint inhibitors (ICIs) various ways. (1) The metabolites produced by host’s homeostasis, these function distribution cells, promote inhibit inflammatory responses, phenotype, angiogenesis, response, cell infiltration GBM thereby affecting effectiveness ICIs. (2) Some members may reverse T inhibition, increase anti-tumor activity, ultimately improve ICIs targeting specific immunosuppressive cytokines. (3) participate metabolic process drugs, which degrade, transform, produce metabolites, effective concentration bioavailability drugs. Optimizing structure help (4) also regulate status through axis communication, progression therapeutic response to (5) Given importance for ICI therapy, researchers have begun exploring use fecal transplantation (FMT) transplant healthy optimized patients, order their enhance therapy. Preliminary studies suggest FMT therapy some patients. Background/Objectives: Glioblastoma (GBM), a highly aggressive grade IV astrocytoma, poses major therapeutic challenge due to the resistance of cancer stem cells (CSCs) existing within its cell population conventional therapies. Recently, we reported that RNA interference targeting CSC protection mechanism significantly improved efficacy. However, challenges remain, including limited transfection efficiency in neural and difficulty crossing blood–brain barrier (BBB). Methods: In this study, investigated potential exosome-mediated delivery cargo GBM by engineering exosomes carry green fluorescent protein (GFP) expressing brain-homing peptide (BHP) on their surface, which has high affinity cells. Results: We found BHP-modified doubled GFP efficacy from 20% 40%, outperforming traditional methods like lipofection vitro. vivo, demonstrated an ability cross BBB targeted brain regions following intranasal subcutaneous administration. Conclusions: These results underscore engineered for efficient enhance against tumors suggest novel avenues delivering biomolecules treatment neurological disorders.

Glioma and glioblastoma multiform (GBM) remain among the most debilitating life-threatening brain tumors. Despite advances in diagnosing approaches, patient follow-up after treatment (surgery chemoradiation) is still challenging for differentiation between tumor progression/recurrence, pseudoprogression, radionecrosis. Radiomics emerges as a promising tool initial diagnosis, grading, survival prediction patients with glioma can help differentiate these post-treatment scenarios. Preliminary published studies are about role of radiomics glioma/GBM. However, this field faces significant challenges, including lack evidence-based solid data, scattering publication, heterogeneity studies, small sample sizes. The present review explores radiomics's capabilities following glioma/GBM status to progression, recurrence,

Glioblastoma (GBM) is the most aggressive brain tumor, characterized by high recurrence rates and poor patient outcomes. Treatment failure driven multiple factors, including complex tumor heterogeneity, presence of cancer stem cells, immunosuppressive microenvironment (TME), many others. GBM’s heterogeneity underlines its ability to resist therapies adapt TME. The TME, which highly shaped hypoxia, impairs anti-tumor immunity limits efficacy immunotherapy. blood–brain barrier (BBB) remains a major obstacle delivering sufficient drug concentrations restricting penetration therapeutic agents. Another problem lack reliable biomarkers perform better stratification or even guide personalized treatments, resulting in generalized approaches that do not adequately address GBM complexities. Glioblastoma, an aggressive and lethal brain tumor, presents enormous clinical challenges, including molecular heterogeneity, high recurrence rates, resistance to conventional therapies, limited therapeutic penetration across the blood-brain barrier. The glioblastoma microenvironment, characterized by a dynamic interplay of cellular non-cellular components, is key driver tumor growth resistance. Neuroinflammatory cytokines, particularly interleukins necrosis factor-alpha, play pivotal roles in this contributing progression immune evasion. This review highlights oncolytic virotherapy as promising avenue, focusing on its potential modulate neuroinflammatory responses, induce localized reactions, deliver immunomodulatory factors directly site. While encouraging outcomes have been observed, challenges such overcoming barrier, managing host antiviral immunity, mitigating risks normal neuronal cells remain critical barriers translation. By analyzing intricate interactions viruses with microenvironment synthesizing findings from preclinical trials, provides actionable insights into developing personalized effective strategies for based alone or when using it combined immunotherapy, natural killer-cell therapy, chimeric antigen receptor-T cell dendritic therapy.

Glioblastoma: a highly aggressive brain tumor, presents substantial challenges in treatment and management, with surgical intervention playing pivotal role improving patient outcomes. Disparities access to tumor surgery arise from multitude of factors, including socioeconomic status, geographical location, healthcare resource allocation. Low- middle-income countries (LMICs) often face significant barriers accessing services, such as shortages specialized neurosurgical expertise, limited infrastructure, financial constraints. Consequently, glioblastoma patients LMICs experience delays diagnosis, suboptimal treatment, poorer clinical outcomes compared high-income (HICs). The impact these disparities is profound. Patients are more likely be diagnosed at advanced disease stages, receive less effective have lower survival rates than their counterparts HICs. Additionally, care exacerbate economic societal burdens, emphasizing the urgent need for targeted interventions health policy reforms address inequities. This review highlights importance addressing global through collaborative efforts, advocacy, allocation, aiming improve promote equity delivery all worldwide.
