Diabetes management requires constant monitoring and individualized adjustments. This study proposes a novel approach that leverages digital twins Personal Health Knowledge Graphs (PHKGs) to revolutionize diabetes care. Our key contribution lies in developing real-time, patient-centric twin framework built on PHKGs. integrates data from diverse sources, adhering HL7 standards enabling seamless information access exchange. PHKGs offer flexible efficient format supports various applications. As new knowledge about the patient becomes available, PHKG can be easily extended incorporate it. dynamic fosters continuous improvement facilitates development of proof concept, we have demonstrated versatility our by applying it different use cases management. These include predicting glucose levels, optimizing insulin dosage, providing personalized lifestyle recommendations, visualizing health data.
