Abstract Ionizing radiation can have a wide range of impacts on tumor–immune interactions, which are being studied with the greatest interest and at an accelerating pace by medical community. Despite its undeniable immunostimulatory potential, it clearly appears that radiotherapy as is prescribed delivered nowadays often alters host's immunity toward suboptimal state. This may impair full recovery sustained efficient antitumor immunosurveillance posttreatment. An emerging concept arising from this awareness consists reconsidering way designing treatment planning, notably taking into account individualized risks deleterious radio-induced immune alteration be deciphered planned beam trajectory through lymphocyte-rich organs. In review, we critically appraise key aspects to consider while planning immunologically fitted radiotherapy, including challenges linked identification new dose constraints immune-rich structures.
