UNSTRUCTURED The benefits and challenges of electronic health records (EHRs) as data sources for clinical epidemiologic research have been well described. However, several factors are important to consider when using EHR study novel, emerging, multifaceted conditions such postacute sequelae SARS-CoV-2 infection or long COVID. In this article, we present opportunities improve our understanding COVID, based on lessons learned from the National Institutes Health (NIH)–funded RECOVER (REsearching COVID Enhance Recovery) Initiative, suggest steps maximize usefulness performing research.

Background: Cardiopulmonary exercise testing (CPET) is a noninvasive and nonexpensive diagnostic tool, that provides comprehensive evaluation of the pulmonary, cardiovascular, skeletal muscle systems’ integrated reactions to exercise. CPET has been extensively used in adults with Long COVID (LC), while evidence about its role children this condition scarce. Methods: Prospective, case-controlled observational study. Children LC control group healthy underwent CPET. findings were compared within 2 groups, groups according main clusters persisting symptoms. Results: Sixty-one 29 controls included. Overall, 90.2% patients (55 61) had pathologic test vs 10.3% (3/29) control. presented statistically significant higher probability having abnormal values peak VO2 ( P = 0.001), AT% pred <0.001), VO2/HR % 0.03), work slope 0.002), VE/VCO2 0.01). The incidence of long COVID in adult survivors an acute SARS-CoV-2 infection is approximately 11%. Of those afflicted, 26% have difficulty with day-to-day activities. majority COIVD cases occur after mild or asymptomatic infection. Children can spread infections and also develop long-term neurological, endocrine (type I diabetes), immunological sequelae. Long COVID affects a significant number of children, yet clinician knowledge gaps and limited access to specialized care hinder effective management. With fewer than 20 pediatric long clinics in the United States, many families must travel distances for care. To address these challenges, ECHO (Extension Community Healthcare Outcomes) program was developed educate health professionals on evidence-based The engaged 94 participants from States Canada via weekly tele-education sessions, recruited through word mouth professional listservs. Pre-surveys (41% response rate) post-surveys (29% were sent attendees. Participants reported statistically improvements knowledge, confidence, competence, self-efficacy ( P < 0.001). This represents valuable initiative facilitate timely interventions empower primary community providers diagnosing, treating, managing populations.

Abstract Background Long COVID is a complex, heterogeneous syndrome affecting over four hundred million people globally. There are few recommendations, and no formal training exists for medical professionals to assist with clinical evaluation management of patients COVID. More research into the pathology, cellular, molecular mechanisms COVID, treatments needed. The goal this work disseminate essential information about recommendations definition, diagnosis, treatment, social issues physicians, researchers, policy makers address escalating global health crisis. Methods A 3-round modified Delphi consensus methodology was distributed internationally 179 healthcare professionals, persons lived experience in 28 countries. Statements were combined specific areas: research, society. Results survey resulted 187 comprehensive statements reaching strongest areas being diagnosis assessment, general research. We establish conditions different subgroups within umbrella. Clear reached that impacts COVID-19 infection on children should be priority, additionally need determine effects societies economies. it affects nervous system other organs not likely observed initial symptoms. note, biomarkers critically needed these issues. Conclusions This forms guidance spectrum as disease reinforces translational large-scale treatment trials protocols.
