The precise and efficient diagnosis of an individual's skeletal class is necessary in orthodontics to ensure correct stable treatment planning. However, it difficult efficiently determine the true due several correlations between various anatomic structures. primary outcome this prospective cross-sectional study was developing a machine learning model for classifying patients as I III. Furthermore, investigation intended compare cephalometric variables III well age sex-specific subgroups analyse parameters perform Principal Component Analysis (PCA) identify most important contributing variances. This based on pre-treatment lateral cephalograms 509 German orthodontic diagnosed (n = 341) or 168) according individualised ANB Panagiotidis Witt, following descriptive analyses parameters, correlation followed by key variables. Machine models, including Random Forest (RF), Classification Regression Trees (CART), k-nearest Neighbors (KNN), Linear Discriminant (LDA), Support Vector Machines (SVM), Generalized Model (GLM), were evaluated accuracy. Within same class, influenced parameters: I, adolescents presented more horizontal pattern (PFH/AFH, Gonial angle, NL-ML) prominent mandible (SNB, SN-Pg) than children. In III, degree sagittal discrepancy jaw bases notable adults (ANB: III_Age > 21-III _14 < Age 20 - 1.78°). Comparing latter had prognathic mandibles (SNB) compensated incisors' inclination (proclination upper (+ 1/NA: 9.01°), retroinclination lower incisors (- 1/ML: 8.99°). Among others, found (degree prognathism, SNB) vertical (inclination, ML-NSL) orientation (skeletal I: p 0.001, ρ 0.742; III: 0.665). PCA revealed that first four principal components explain 93% variance I/III these influence loading score component-PFH/AFH ratio (0.35), SNB angle SN-Pg (0.37), ML-NSL 0.35). Evaluating general model, all age, sex, resulted perfect (1.00) accuracy kappa scores compared gold standard Calculated_ANB with model's RF CART. 2 amount input reduced (Wits, only), but (0.88), (0.73) still good KNN model. last section study, we applied different classification models. We examined ability parameters-SNA, SNB, angles predict results demonstrated GLM gained 0.99 (Accuracy 0.99, Kappa 0.97). can be simplified applying SNA, only. stresses importance their identification. considering classes, larger population needed validate generalize approach.
