Background The association of prediabetes and diabetes on health-related quality life (HRQoL) remains inconclusive in current epidemiological research. In this investigation, we administered 5-level EuroQoL-5 dimension version (EQ-5D-5L) to systematically assess HRQoL across glycemic strata (diabetes, prediabetes, normal levels) Guangzhou, offer baseline data that can be easily compared other regions China or countries. Method This investigation utilized cross-sectional extracted from a three-year prospective cohort study conducted at the Health Management Center Guangzhou 11th People’s Hospital. Propensity score matching was implemented 1:1:4 ratio balance participants diabetes, group. outcomes, operationalized through EQ-Index EQ visual analog scale (EQ-VAS) measurements, were groups using one-way ANOVA Wilcoxon rank-sum tests. Multivariate linear regression constructed adjust for potential confounders, followed by subgroup analyses stratified sex, age categories, body mass index (BMI) classifications, hypertension comorbidity status. Results A total 18,605 included study. After propensity matching, 533 allocated group, 2064 Intergroup comparisons demonstrated significantly lower EQ-VAS scores group (79.11) both (80.67) (81.65). Similarly, exhibited lowest (0.968) relative (0.972) (0.972). adjusted with age, BMI, etc. revealed 2.139-point reduction versus (95% CI: -3.748, -0.530; P =0.009). Subgroup identified particularly compromised populations among female participants, individuals obesity, those aged ≥60 years. Discussion Prediabetes mellitus are associated diminished levels, more pronounced negative associations observed populations, older adults, obesity. These findings emphasize clinical necessity implementing targeted interventions optimize outcomes these high-risk subgroups, which aligns fundamental objectives contemporary management frameworks. Background China's rapidly aging population necessitates effective strategies for ensuring older adults' quality of life (QOL). While individual factors (IF) and family (FF) are known to influence QOL, existing research often examines these in isolation or focuses on specific subpopulations, overlooking potential interactions mediating pathways. Objective This study aims examine both direct indirect pathways connecting IF FF adults’ focusing the roles health risks (HR) care service demand (HSD). Methods uses structural equation modeling (SEM) analyze cross-sectional data from 8600 participants 2015 China Health Retirement Longitudinal Study (CHARLS), a nationally representative using multistage probability proportional size sampling method. Results Among participants, majority (5586/8502, 65.7%) were aged 60-70 years, with near-equal distribution males females at around 50%. The average PCS score was 76.77, while MCS averaged 59.70. Both (β=0.165, P<.001) (β=0.189, had positive effect QOL. Furthermore, effects (β=0.186, (β=0.211, through HR HSD also significant. In model, greater impact (β=0.841) than (β=0.639). However, after including 2 factors, HSD, (β=0.739) became consistent that (β=0.728). Subgroup analyses revealed QOL significant age group (β=0.151, but not those over 70 years old (β=0.122, P=.074). Comorbidity status significantly influenced pathway adults having more chronic diseases (β=0.363) showing compared fewer (β=0.358). Conclusions (education, per capita disposable income, endowment insurance) (satisfaction spouse children) directly people. Meanwhile, have equal role HSD. Recognizing interplay among is crucial targeted interventions enhance well-being China.
