In this narrative review, we sought to provide a comprehensive overview of the mechanisms underlying cutaneous senescence, framed by twelve traditional hallmarks aging. These include genomic instability, telomere attrition, epigenetic alterations, loss proteostasis, impaired macroautophagy, deregulated nutrient sensing, mitochondrial dysfunction, cellular stem cell exhaustion, altered intercellular communication, chronic inflammation, and dysbiosis. Photoaging is a process related to an increased level of reactive oxygen species (ROS). Polyphenols can scavenge free radicals in the body, which delay skin aging. Lactic acid bacteria (LAB), a probiotic, provide various health benefits. We recently isolated new Lactobacillus paracasei strain with strong anti-inflammatory effects under lipopolysaccharide-induced conditions and proposed mode of action—augmenting the endoplasmic reticulum stress pathway for functions in host cells. Background and Objectives: Actinic keratosis (AK) is a precancerous cutaneous lesion driven by chronic ultraviolet (UV) exposure, often coexisting with features of photoaging, such as wrinkles pigmentary irregularities. Recent evidence suggests that treatments for AK may also counteract photoaging through shared molecular pathways, including oxidative stress inflammation. This narrative review explores the dual benefits therapies, highlighting their potential anti-aging skin-lightening effects, implications improving skin appearance alongside clearance. Materials Methods: The literature was analyzed to assess efficacy, mechanisms, cosmetic outcomes commonly used treatments, topical agents (5-fluorouracil (5-FU), imiquimod, diclofenac, tirbanibulin), photodynamic therapy (PDT). Studies effects on photoaged skin, collagen remodeling, pigmentation, patient satisfaction were reviewed. Results: PDT emerged most validated treatment, demonstrating improved synthesis, texture, pigmentation. 5-FU showed remodeling dermal matrix increased procollagen levels, but local reactions represent major limitation. Imiquimod enhanced fibroplasia reduced solar elastosis, while diclofenac provided mild photodamage improvements minimal adverse effects.
